Objective:To explore diabetes and risk of 30-days and 1-year outcomes after fracture and 1-year mortality in elderly patients with fragility hip fracture.Methods:A 1:3 matched nest case-control study paired by age and sex was conducted.A total of 644 fragility hip fracture patients treated at the First Affiliated Hospital of Chongqing Medical University between June 2013 and July 2016 were recruited in our study.The clinical characteristics,short term outcomes and 1-year outcomes after fracture and 1-year mortality for 161 diabetic hip fracture patients were comparatively analysed with 483 non-diabetic patients.Results:1)161(25%)cases were enrolled into diabetes group,483(75%)cases were enrolled into non-diabetes group.2)Diabetic patients were more likely to be nonsmokers and have higher serum albumin,poorer renal function,more comorbidity,higher costs,longer days awaiting surgery and longer length of hospital stay(P<0.05).3)Compared with non-diabetics,diabetic patients are more prone to developing urinary tract infections and deep vein thrombosis complications(P<0.05).4)At one-year follow-up,no differences in recovery of function and mortality were observed between the two groups.Conclusion:Diabetics are at an increased risk of specific complications and have a longer length of hospital stay and higher cost but have similar recovery of function and 1-year mortality compared to non-diabetics.More research is needed on the causes underlying such excess risk and on the effectiveness of measures to reduce post-fracture complications in diabetic subjects.
