ObjectiveTo evaluate right ventricular alternative site pacing on left ventricular systolic function and synchronicity by two-dimensional speckle tracking imaging and real time three dimensional, and investigate the correlation of two techniques.Materials and Method27patients wiht permanent pacemaker implantation were divided into2 groups according to different pacing site:RVA pacing and RVOT pacing.LVEF、longitudinal strain、left ventricular systolic synchronicity were evaluated by 2D-STI and RT-3DE before and 1 month after implantation, and analyse the correlation of Tp-SD and Tmsv-16-SD%.ResultLVEF was significantly decreased postoperatively in both groups.Compared with preperative, the peak systolic longitudinal strain ofLV apical segment was significantly increased 1 month postoperatively in RVA pacing(P<0.05).Left ventricular systolic synchronicities were significantly increased postoperatively in RVA and RVOT pacing(P<0.05),and part of the synchrony parameters were significantly increased in RVA pacing than in RVOT pacing(P<0.05).Tp-SD and Tmsv-16-SD% have positive correlation(r=0.775,P<0.001).ConclusionSystolic function of left ventricular was decreased 1 month postoperatively in both groups. Both RVA and RVOT pacing may induce left ventricular systolic synchrony, but RVOT pacing is more beneficial in left ventricular systolic synchronicity than RVA pacing. Both 2D-STI and RT-3DE can assess LV systolic synchronicity, and have positive correlation.
