Readmission Of Children With Bronchopulmonary Dysplasia In The First 2 Years Of Life:A Clinical Analysis Of 121 Cases

Objective: To analyze the clinical features and treatments of readmitted among infants with bronchopulmonary dysplasia(BPD)in the first 2 years of life,to investigate the effect of BPD on the prognosis of respiratory diseases in premature infants,and to help improve the diagnosis and treatment of children with BPD who are readmitted due to lower respiratory tract infection.Methods: A retrospective analysis was performed on the clinical data of 242 cases readmission which were caused by lower respiratory tract infection of 121 children with BPD within 2 years old.Results : 1.In 242 cases,115 cases(47.5%)had wheezing,the incidence of wheezing in the second year is significantly increased than the first year of life.2.31(16.1%)cases showed hyperlucent areas in 193 cases chest images.The levels of TV/kg,TPEF/TE,VPEF/VE,TEF50,TEF75 and RR in 39 cases BPD group who take pulmonary function test had significant differences with those who had no respiratorydisease(P<0.05).21(75%)out of 28 cases who had been performed fiber bronchoscope had airway dysplasia.One lung biopsy shows thickened alveolar walls and dysplasia of the alveolar capillaries.3.There were no treatment-related side effects occurred in 242 cases who had used inhaled corticosteroids(ICS);6 cases who received intravenous human umbilical cord mesenchymal stem cells(hUC-MSCs)infusion had no infusion related adverse reactions;moreover,One of them successfully stopped the oxygen therapy.Conclusion: The readmission of children who have BPD with lower respiratory tract infection decreases with the growth of the age.But the incidence of wheezing increases significantly among these children.The pulmonary function mainly showed small airway obstruction,slower expiratory flow rate in less lung capacity and accelerating respiratory frequency.Bronchoscope found airway dysplasia most frequently,mainly composed of bronchial stenosis.There is no exact effective treatment for re-admitted children with BPD,ICS might be a better choice to inhibit acute inflammation response.Meanwhile,hUC-MSCs is proved to be safe and feasible in short-term treatment,which might have benefits for BPD's recovery and provide a new method for BPD's therapy.
