| Objective: To analyze the risk factors for post Endoscopic RetrogradeCholangiopancreatography pancreatis and hyperamylasemia while assessing the effect of prophylactic application of epinephrineMethods:This study was selected from the Second Hospital of Hebei Medical University from January 2017 to January 2018,with 239 patients who were diagnosed as choledocholithias is in the biliary minimally invas ive procedure of ERCP.Design forms and collect relevant information.To determine the risk factors,independent risk factors and the effect of epine-phrine in the postoperative pancreatitis and hyperamylasemia inc idence after ERCP.The related risk factors were analyzed by single factor,and multivariate logistic regression analysis was performed on the factors with statistical significance(P<0.05).Results: In 239 cases of patients,9 cases of patients with postoperative ERCP pancreatitis,the incidence of PEP was 3.76%(9/239),76 cases of patients were clinical diagnosis of postoperative ERCP hyperamylasemia,rate of 31.79%(76/239).The PEP single factor analysis was used to analyze the risk factors for the basket accesses to the pancreatic duct,the diameter of the stone in common bile duct(less than 0.6cm)and the difficult intubation(more than 10 minutes),adrenaline is the protective factor.Multiple factors of PEP were analyzed to determine the independent risk factors for the basket accesses the pancreatic duct and the diameter of the stone in common bile duct(less than 0.6cm).The single factor for hyperamylasemia after ERCP in the analys is of gender,difficulty intubation(more than 10 minutes),and operation time(30 minutes or more)are risk factors.Multi-factor analys is operation time(30 minutes or less)is an independent risk factor.Conclusion:Prevention of the use of 1:10,000 epinephrine as PEP protective factor;The diameter(less than 0.6cm)of the stone in the common bile duct and the basket accesses to the pancreatic duct were the independent risk factors of PEP.Operation time(30 minutes or more)was an independent risk factor for hyperamylasemia after ERCP. |
