Objective: Through summarizing ultrasound image characteristics of different types of aortic arch anomalies from three vessels trachea view,long axis view of the aortic arch and coronal view of the descending aortic arch,we discuss the value of these views to aortic arch anomalies,in order to improve the detection rate of prenatal aortic arch anomalies,to investigate the clinical value of fetal echocardiography in evaluation of aortic arch anomalies diagnosis and prognosis.Methods: A retrospective analysis of 215 cases of fetal echocardiography and clinical data that were fetal aortic arch anomalies and confirmed after delivery from January 2016 to October 2017 at Children's Hospital of Hebei Province and Hebei Maternity Hospital.The aortic arch anomalies were divided into three groups: congenital vascular ring,coarctation of the aorta and interruption of aortic arch.The three section diagnosis method for cases of fetal heart examination,routine fetal heart check suspicious aortic arch deformity,should focus on the scanning of aortic arch,the coronal section of three vessels and trachea view and long axis view of aortic arch,every cases were collected and stored in a dynamic graph,hard disk machine.Prenatal diagnosis of aortic arch anomalies should follow up the children's echocardiography around 3 months after delivery,if not,telephone follow-up can be made.Results:1.A total of 215 cases of aortic arch malformation were diagnosed by fetal echocardiography,144 cases of live birth,37 cases of termination of pregnancy and 34 cases of lost visit.The aortic arch deformity accounted for 17.5%(215/1229)of all congenital cardiovascular malformations detected.The total live births in 144 cases,141 cases of postpartum follow-up for correct diagnosis,missed diagnosis in 1 cases,2 cases were misdiagnosed,super aortic arch deformity prenatal diagnosis coincidence rate was 97.9%(141/144),the right aortic arch,left ductus aberrant left subclavian artery prenatal diagnosis rate was 98.6%,aortic arch contraction narrow prenatal diagnosis rate was 83.3%,right aortic arch and left ductus with mirror branch,right aortic arch and right artery catheter,prenatal double aortic arch coincidence rate was 100%.2.In the detection of aortic arch malformation,congenital vascular rings were the most common(178 cases),accounting for 82.8%(178/215),22 cases of aortic arch constriction,10.2%(22/215),and 15 cases of aortic arch rupture,accounting for 7%(15/215).Of which 178 cases of congenital vascular rings,right aortic arch and left ductus with aberrant left subclavian artery in 96 cases,accounting for 53.9%(96/178),right aortic arch and left branch,mirror artery catheter in 26 cases,accounting for 14.6%(26/178),6 cases of double aortic arch,3.4%(6/178),4 cases of right aortic arch artery catheter,accounted for 2.2%(4/178),right subclavian artery vagus in 46 cases,accounting for 25.8%(46/178).3.The rate of displaying the three vessel trachea section of congenital aortic arch is 99.1%(210/215),the rate of aortic arch long axis is 94.6%,and the rate of coronary artery section is 74.2%.Conclusion:1.Three vessels trachea view is the main view to screen the aortic arch anomalies,for the view is of high display rate.2.Coronal view of the descending aortic arch can distinguish double aortic arch from mirror-image right aortic arch,and also can show aberrant subclavian artery well3.It's important for long axis view of the aortic arch and coronal view of the descending aortic arch to diagnose coarctation of aorta.
