Objective:To analyze the efficacy of endovascular treatment of thoracic aortic dilatation disease in the reconstruction of aortic arch branch vessel revascularization.METHODS: From January 2012 to April 2017,thoracic aortic dissection and thoracic aortic aneurysm were performed in Department of Vascular Surgery,First Affiliated Hospital of Guangxi Medical University.The aortic dissection was performed.A total of 22 patients were treated with recanalization of the branch vessels of the bow.Among them,TEVAR combined with chimney technology in 10 cases,TEVAR combined with window technology in 9 cases,TEVAR combined with cervical vascular bypass in 3 cases.To preoperative basic treatment,thoracic aortic stent graft implantation combined with different methods to rebuild the branches of the branch vessels,and postoperative treatment.The clinical data of the cases were retrospectively analyzed.Preoperative,postoperative 1 week,postoperative 1 month,3 months,6 months,and 12 months were taken as time points to collect and collate the clinical data of the patients and evaluate the treatment effects.Observe postoperative complications.Results: All 22 patients were successfully operated.There was no death within 1week after surgery.One patient in the chimney group and neck vascular bypass group died 1 month after surgery.A total of 6 patients developed endoleak after operation,and 2 patients developed new cerebral infarction lesions.One patient had no clinical symptoms,one patient had an angle relief,and his symptoms improved after conservative treatment.One patient had steal syndrome and no intraoperative death occurred.Postoperative paraplegia and severe cerebrovascular accidents.There were no new deaths during the follow-up period from one month after surgery to one year after surgery,and new tears were formed by reverse tear.All stents or artificial blood vessels were smooth in all cases,and no obstruction or stenosis occurred during follow-up.
