?Subject? Study of the relationship between the level of serum ferritin(SF)and its change in pregnancy and gestational diabetes mellitus.?Methods?From December 2015 to September 2017,144 cases of pregnant wo men with gestational diabetes were selected as the study group,while 234 pregn ant women without pregnancy complications at the same time were selected as control group.All those women did regular obstetric examinationin the Shengj ing Affliated Hospital of China Medical University.The levels of the SF,hemo globin and hematocrit in early pregnancy of two groups were compared.The l evels of the SF in the mid trimester of pregnancy and its changes in two grou ps were also compared.The association between the levels of SF and its chan ges and GDM was analyzed.?Results?In the first trimester,the SF level was 53.20±45.82 ng/ml in the g estational diabetes group and 41.74±28.32 ng/ml in the control group,there w as significant differences between them(P<0.05).In the second trimester,the SF level was 34.52±30.20 ng/ml in the gestational diabetes group and 28.02±21.97 ng/ml in the control group,there was significant differences between t hem(P<0.05).To compare SF changes from first trimester to the second tri mester was 20.12±16.50 ng/ml in the gestational diabetes group and was 14.72±18.16 ng/ml in the control group,there was statistical difference between th em(P<0.05).ROC curve analysis showed the area under the SF curve of th e first trimester(AUC)was 0.622,on this curve,the maximum of Youden Inde x was 0.232.The corresponding SF cut-off value was 40.55ng/ml.ROC curve a nalysis showed the area under the SF curve of the second trimester(AUC)w as 0.566,on this curve,the maximum of Youden Index was 0.151.The correspo nding SF cut-off value was 37.10ng/ml.The Logistic analysis was performed o n the related factors of GDM.The results indicated when the SF was greater t han 40.55ng/ml the OR value was 2.832(P<0.05)in the first trimester.When the SF was greater than 37.10ng/ml,the OR value was 2.433(P<0.05)in the second trimester of pregnancy.?Conclusions?The SF levels in the first and the second trimester of pregnanc y and the change of SF from the first to the second trimester of pregnancy w ere correlated with the incidence of GDM.The GDM risk of first trimester pr egnancy whose SF level greater than 40.55ng/ml was 2.832 times higher than t hose with lower SF level.The GDM risk of second trimester pregnancy whose SF level greater than 37.10ng/ml was 2.433 times higher than those with low er SF level.SF levels in the first and the second trimester might be used to a nticipate the incidence of GDM.
