0bjective:Explore the influence in 3D translabial ultrasound for anal sphincter complex and measurements in primiparous women delivering by vaginal birth versus cesarean delivery.Methods: 157 primiparous women underwent 2D and 3D translabial ultrasound after deliveing 42 days.We measured internal anal spincter at 12,3,6,9 o'clock at proximal,mid and distal level,external anal sphincter at 12,3,6,9 o'clock at distal level,and puborectalis muscle at 4 and 8 o'clock at mid level.Evaluate the shape and integrity of anal sphincter complex using tomographic ultrasound imaging and volume contrast imaging.Result:There were 157 primiparous women after delivering,77 underwent vaginal birth and 80 underwent cesarean delivery,36 women underwent lateral episiotomy,and 1woman underwent forceps.10 women have different degrees of abnormal symptoms of defecation and 3 women have symptoms of fecal incontinence.Measurements of external anal sphincter at distal 12 o'clock position,internal anal sphincter at proximal 12,6 o'clock position and mid 12,9 o'clock position were significally thinner for vaginal birth women(p<0.05),We also observed 1 woman had external anal sphincter defect.Conclusion:There are sigmificant difference between vaginal birth and cesarean delivery:For vaginal birth women,measurements of external anal sphincter at distal 12 o'clock position,internal anal sphincter at proximal 12,6 o'clock position and mid 12,9 o'clock position were significant thinner than cesarean delivery women.3D translabial ultrasound can clearly observe the shape of anal sphincter complex and diagnose defect,has high value of evaluating postpartum anal sphincter complex.
