Objective: To assess the prevalence of and related risk factors for aspirin resistance in patients with cerebral infarction.To explore the relationship between aspirin resistance(AR)and glycated albumin(GA),and to find biochemical markers of aspirin resistance in patients with cerebral infarction.Methods: 109 patients with cerebral infarction who received daily aspirin therapy after admission were recruited.The platelet aggregation rate was determined by light transmission aggregometry(LTA),using adenosine diphosphate(ADP)and arachidonic acid(AA)as inducers.Aspirin resistance was defined as ≥ 70% adenosine diphosphate(ADP)-induced and(or)≥ 20% arachidonic acid(AA)-induced aggregation.The value of glycated albumin is determined by enzymatic method.Patients were divided into three groups according to whether there was a recurrence of cerebral infarction.Based on the results of platelet aggregation rate and GA,patients were divided into two groups respectively.The prevalence of AR and its risk factors were analyzed statistically.The relationship between GA and AR is further discussed.Results: The prevalence of AR is 34.86%(38/109)in our study.the group of acute recurrent cerebral infarction.The incidence of AR in the group of acute recurrent cerebral infarction was higher than that of Initial acute cerebral infarction group(c2=13.410,P<0.05).Patients with diabetes are more likely to develop AR(c2=5.991,P<0.05).The level of GA,fasting plasma glucose and glycosylated hemoglobin in AR group were significantly higher than those in the AS group(P<0.05).The prevalence of aspirin resistance in GA elevated group was higher than that in GA normal group,and the platelet aggregation rate is also higher relatively(P <0.05).Multivariate logistic regression analysis revealed that GA(Odds ratio: 1.226;95%CI: 1.008-1.490;P = 0.041)was a independent risk factor for AR.GA,fasting plasma glucose and glycosylated hemoglobin may be the predictive indicators of AR in patients with cerebral infarction(P=0.000,0.002,0.008).Conclusions: A part of patients with cerebral infarction will have AR,and the recurrence of cerebral infarction may be related to AR.GA is closely associated with aspirin resistance in patients with cerebral infarction,and an independent risk factor for AR.GAmay be a predictive indicator of AR in patients with cerebral infarction.Therefore,patients with cerebral infarction should actively control the level of GA and reduce the incidence of AR. |
