Objective:Analyzing the risk factors that affect bleeding complications after renal biopsy provides a clinical basis for preoperative risk assessment of renal biopsy.Methods:Selected from January 2006 to December 2016 in the Department of Nephrology,Nanking District,Shengjing Hospital Affiliated to China Medical University,2546 patients were eligible for renal biopsy and performed renal biopsy.The patient's clinical data were collected,including age,sex,weight,history of hypertension,blood pressure(systolic blood pressure,diastolic blood pressure),hemoglobin and hematocrit,platelet count,serum creatinine,e GFR,PT,APTT,FIB,Hemoglobin and hematocrit after biopsy,as well as histological diagnosis after renal biopsy.According to the definition of bleeding complications after renal biopsy,they were grouped and retrospectively analyzed risk factors affecting bleeding complications after renal biopsy.Results:1.Among 2546 renal biopsy patients,2399 patients(94.2%)had no bleeding complications,147 patients(5.8%)had bleeding complications.There were 75patients(3.0%)had mild bleeding,39 patients(1.5%)with moderate bleeding and 33 patients(1.3%)with severe bleeding.2.Comparison of clinical data of different degrees of bleeding complications: there was no statistically significant difference between groups in weight,with or without hypertension(P>0.05),but there was statistical difference between groups in gender,age,Scr,and e GFR(P < 0.05).3.Comparison of pathological types between different degrees of bleeding complications:there was no statistically significant difference between groups in mesangial proliferative nephropathy,diabetic nephropathy,purpura nephritis,crescentic glomerulonephritis,minimal change glomerulonephritis,focal segmental glomerulonephritis nephritis,hyperplastic sclerosing glomerulonephritis,Ig M nephropathy,acute interstitial nephritis,lupus nephritis,membranoproliferative glomerulonephritis,hepatitis B associated nephritis,capillary proliferative glomerulonephritis,thrombotic microangiopathy,acute tubular injury,acute tubular injury,ANCA-associated systemic vasculitis,kidney amyloidosisand and other pathological type(P>0.05).There was statistical difference between groups in Ig A nephropathy,membranous nephropathy,hypertensive nephropathy and systemic scleroderma renal crisis(P < 0.05).4.Scr(P<0.05)and hypertensive nephropathy(P<0.001)affected independent risk factors for bleeding complications after renal biopsy.Conclusions:1.The overall incidence of bleeding complications after renal biopsy is not high,especially the incidence of severe bleeding complications was 1.3%,and no further damage was caused by treatment,so percutaneous renal biopsy is a relatively safe clinical examination.2.Scr and hypertensive nephropathy were related to bleeding complications after renal biopsy.Increased Scr and pathological type of hypertensive nephropathy were risk factors for bleeding complications after renal biopsy.
