Objective: Retrospective analysis of the clinical characteristics and risk factors of 76 patients with sepsis-associated liver injury caused by abdominal infection.Methods: The clinical data of patients with sepsis caused by abdominal infection were collected from the ICU of the First Hospital of the China Medical University,in January 2016-June 2017.It is divided into sepsis associated liver injury group and non-liver injury group.SPSS22.0 software was used for descriptive statistics.Because of the normality or homogeneity of variance of the two sets of data,Mann-Whitney U test is used to compare two groups patients with age,length of stay in ICU,APACHEII score,SOFA score.Basic diseases,whether or not mechanical ventilation,whether or not CRRT,whether the blood the positive culture,whether in septic shock,and the death during ICU are compared with X2 test.These factors that may affect the liver dysfunction use stepwise logistic regression..Results: A total of 223 patients with sepsis caused by abdominal infections were included in this study.44 patients were excluded according to the exclusion criteria,of which 76 cases were liver injury,21 died and 55 survived.In 103 cases of non liver injury group,12 cases died and 91 cases survived.There was no significant difference in APACHEII score,gender,basic disease,mechanical ventilation and CRRT between the two groups.There were significant differences in age,ICU time,SOFA score,bacteremia,septic shock and death during ICU(P<0.05)in the two groups.The average stay ICU time of the liver injury group was 27.9 + 12.43 days,the average SOFA score was 6.53 + 3.04,the average stay ICU time of the non liver injury group was 6.17 + 9 days,the average SOFA score was 5.41 + 2.46,and the liver injury groups were all higher than those of the non liver injury group.In liver injury group32% had bacteremia,72% had septic shock,28% died;in non-liver injury group,15%had bacteremia,49% had septic shock,12% died,and the liver injury group was significantly higher than the non-liver injury group.Use stepwise logistic regression analysis the factors that may affect liver injury,such as age,ICU time,SOFA score,bacteremia and septic shock.The results suggest that the existence of bacteremia and septic shock are the high risk factors of septic liver injury(P<0.05).Conclusion: The stay ICU time of septic liver injury patients was significantly prolonged by abdominal infection.The SOFA score was significantly higher than that of non liver injury patients,and the mortality in ICU was also significantly higher.Septic shock and bacteremia are high risk factors of sepsis-associated liver injury caused by abdominal infectionn.
