Therapeutic Effect Of Wuling Capsule On Type 2 Diabetes Mellitus With Depression(Disharmony Between Heart And Kidney)and Its Mechanism

Objective:To observe and explore the clinical effect of Wuling capsule on type 2diabetes mellitus complicated with depression,and its effects on chronic inflammatory factors、oxidative stress levels、oninsulin resistance and islet function.Methods : Choosing70 patients with type 2 diabetes mellitus combined with depression that meet the standards.Using the randomized double-blind method to divide the patients into treatment group and placebo group.There are36 patients in the treatment group(The final group was 32)that taking Wuling capsule three times a day and 3 capsules at a time(after meals),they are all in the basic intervention of type 2 diabetes mellitus.There are34 patients in the placebo group(The final group was 32)that taking placebo three times a day and 3 capsules at a time(after meals),they are all in the basic intervention of type 2 diabetes mellitus.After 12 weeks of treatment,the changes of fasting blood glucose(FPG),2 hour postprandial blood glucose(2 h PGN),glycosylated hemoglobin(HbAlc),Hamilton score,inflammatory factors(TNF-αand IL-6),oxidative stress levels(SOD and MDA),insulin resistance and islet B cell function were compared between the two groups.Finally,the effectiveness and therapeutic mechanism of Wuling capsule were evaluated and analyzed.Results : 1.The mean of the Hamilton depression scale(HAMD)in the treatment group and placebo group respectively decreased significantly after 12 weeks of treatment(P<0.05),the difference in the level of the two groups after intervention has statistical significance(P<0.05),and the treatment group is better than placebo group;2.After treatment,HbA1 c was significantly improved in the treatment group(P < 0.05).No significant improvement in HbA1 c in placebo group.Compared with the control group,the FPG of the experimental group was significantly lower than that of the control group,and the difference was statistically significant(P<0.05).3.After 12 weeks of intervention,the levels of IL-6 and TNF-αwere significantly decreased in both groups(P < 0.05).The levels of IL-6 and TNF-αinthe test group were significantly lower than those in the control group after treatment(P < 0.05);4.After 12 weeks of intervention,the SOD levels of both groups increased significantly and the level of MDA decreased significantly(P < 0.05).Compared with the control group,the SOD level of the test group was significantly higher than that in the control group(P<0.05).The level of MDA was significantly lower than that in the control group(P < 0.05).5.After 12 weeks of intervention,the HOMA-β levels of both groups increased significantly(P < 0.05),HOMA-IR and ISI hane improved significantly in the treatment group(P < 0.05),the comparison of HOMA-β 、HOMA-IR and ISI between the treatment group and the placebo group was not statistically significant(P > 0.05),the treatment group is better than the placebo group.Conclusion:Wuling capsule has the effect that lower the HAMD obviously and ameliorate depression of patients.It can also improve the patient’s level of inflammation,oxidative stress state.It has a certain auxiliary effect on the control of blood sugar and islet B cell function recovery.
