Objective: The general and short-term curative effects of PFNA in the treatment of intertrochanteric fractures in the elderly were studied by comparing the results of the study with the slope position and the traditional supine position.The clinical basis was provided for the improvement of PFNA internal fixation and the clinical treatment effect was optimized.Methods: fIn elderly patients with intertrochanteric fractures admitted to our department of orthopedics from September 2015 to December 2017,those who met the inclusion criteria were treated with random posture(slope or supine position)to perform proximal femoral nailing.After PFNA internal fixation,14 patients were randomly selected for each of the two positions after exclusion screening.A total of 28 patients were studied for comparison.14 patients were divided into two groupsaccording to the date of the surgery.Intragroup comparison was divided into "first 7 cases" and "post 7 cases".The time of operation,the length of time for PFNA insertion,the position of the placement of the guide wire in the head and neck,and the use of C during the operation were observed.Number of arm illuminations,intraoperative blood loss,perioperative blood loss,hip function score,Charnley score,etc.at the first,second,third,sixth,and 12 months postoperatively Observe the indicators,use the independent sample t-test for differential typing,and objectively evaluate the differences between the two groups and their significance.Results: Both groups achieved clinical healing and bony union at the final follow-up.The hip function of all affected patients was close to normal.At each follow-up,there was no statistical difference in Harris score and Charnley score between the two groups.The differences between the two groups were statistically significant(P<0.05).The slope group was better than the supine group.There was no statistically significant difference between the two groups in the observations(P>0.05).Conclusions: Performing PFNA internal fixation on the slope position can not only provide the operator with a wideroperating space,but also “offset” the femoral neck anteversion angle,thereby effectively reducing the number of intraoperative fluoroscopy,operation time,perioperative bleeding,and achieve The same long-term efficacy of the traditional supine position is an effective and easy-to-learn surgical position based on the PFNA traditional posture.
