The Clinical Study Of Proximal Femoral Nail For Treatment Of Femoral Intertrochanteric Fracture

Objective To explore the clinical effections and methods in the treatment of intertrochanteric femur fracture by the proximal femoral nail (PFN). For future researeh to provide a basis and to promote a systematic summary of the internal fixation of clinical experience.Methods Retrospective analysis44cases from January2009to December2010, using the proximal femoral nail to treat the intertrochanteric femur fracture. Included males20cases, females24cases.The average age was66.5years (range:21～92). According Evans classification standard, Ⅰ:4cases, Ⅱ:11eases, Ⅲ:12eases, Ⅳ:14eases,V:3eases.Observation of the patients operative time,bleeding situation,postoperative complications, hopitalization days,follow up of patients with fracture healing time,the use of Harris hip score criteria to assess risk of hip function,assessment of therapeutic effect PFN.Results38patients were followed for5～30months and all fractures obtained bony union.According Harris hip function classification standard, The mean Harris hip score was94.8(range:67～96), excellent33cases,86.8%,fine3cases,8.0%,general1case,2.6%.inferior1cases,2.6%.Postoperative proximal locking screw cut whirlbone1ease,Coxa vara1ease, Popliteal vein thrombosis1ease. No broken nail cases.Conclusions In this study,all types of intertroehanterie fracture can be applied by the proximal femoral nail,Its advantage small incision,less trauma,simple mani-pulation,fast recovery and easy removal in the treatment of intertrochanteric femur fracture, which provides reliable effect, it is worth for clinical use.
