Objective: Through observation changes of serum cardiac fatty acid binding protein(HFABP)in patients with acute kidney injury(AKI),to explore the clinical significance of serum HFABP in the diagnosis of AKI.Methods:92 patients diagnosed as acute kidney injury in the First Affiliated Hospital of Shihezi University medical school were collected and divided into AKI stage I group 53 cases,stage II group 26 cases and stage III group 13 cases;At the same time,30 cases of chronic kidney disease(CKD)patients(stage I-IV)and 30 healthy subjects were collected for the same period as the control group.Collect the patients such as age and gender based data,take Serum samples in 24 h after diagnosis,detection of serum creatinine biochemical analyzer(Serum creatinine,SCr,serum cystatin Cystatin C,s Cys C)and other biochemical indicators,using enzyme-linked immunosorbent test(ELISA)to detect the level of HFABP.The levels of SCr,s Cys C and HFABP in AKI I group,AKI II + III group,CKD group and normal group were analyzed.The changes of SCr,s Cys C and HFABP levels in AKI phase I group,phase II group and stage III group were analyzed.Spearman correlation analysis was used to analyze the correlation between HFABP and s Cys C.The clinical significance of HFABP detection in AKI was analyzed by using the receiver operating characteristic(ROC).Results: 1.Comparison of HFABP and s Cys C levels in each group and each period: compared with the normal group,AKI I group and AKI II + III group HFABP levels were higher than normal group(P < 0.05);the level of s Cys C in AKI I group was higher than that of normal group(P < 0.05);compared with CKD group,HFABP level of AKI I group was slightly lower than that of CKD group(P > 0.05),and AKI II + III group HFABP was higher than CKD group(P < 0.05);s Cys C level in AKI I group was slightly lower than CKD group(P > 0.05),and AKI II + III group s Cys C was slightly higher than CKD group(P > 0.05).2.correlation analysis showed that there was a positive correlation between HFABP and s Cys C,and the correlation coefficient was r=0.821,P < 0.05.3.ROC curve shows: AKI I group patients,the AUC of HFABP and s Cys C in the diagnosis of AKI was 0.771(95%CI:0.670-0.871)and 0.733(95%CI:0.627-0.839).The optimal cut-off value of HFABP for diagnosis of AKI was 15.16ng/ml,the sensitivity and specificity were 71.7%,62.1%,respectively;The AUC of HFABP and s Cys C was 0.935(95%CI:0.883-0.987)and 0.918(95%CI:0.858-0.979)in AKI II + III group.The optimal cut-off value of HFABP was 20.12ng/ml,the sensitivity and specificity were 82.1%,86.7%.Conclusion:(1)Serum HFABP has important clinical significance in the diagnosis of AKI,especially the prediction value of AKI II + III group is higher.(2)Serum HFABP level also has certain clinical significance for AKI staging.
