Analysis Of Blood Pressure Variability In Patients With Essential-hypertension From Community Health Service Center

Objective: To analyze the characteristics of blood pressure variability in patients with essential hypertension from community health service center.To provide some thoughts for the diagnosis and treatment of patients with essential hypertension in community work.Methods: A total of 130 patients with essential hypertension come from Shibei district,Qingdao,yan an road in community outpatient clinic and were followed up for more than 3months.Measuring changes in blood pressure and record it per month,Record the change of the patient's condition and the condition of taking medicine.During the follow-up period,the patient experienced changes in cardiovascular and cerebrovascular diseases or other serious diseases,and the patient went to the superior hospital,and the doctor and the patient maintained the clinic and telephone contact.The coefficient of variation(CV)of blood pressure can represent the blood pressure variability(BPV),calculated CV of follow-up systolic blood pressure(SBP)in hypertension patients,take the median CV which can be divided into two groups,low variation group and high variation group.To group the hypertension patients according to complications(Cerebrovascular disease,coronary heart disease,arrhythmia,diabetes,Renal damage)and average of systolic blood pressure and drug utilization.To analyze the characteristics of BPV in patients with essential hypertension from community health service center.Results:(1)Between difficient groups in patients with essential hypertension,there were no statistical significiants in age,gender,BMI and so on(P<0.05);(2)There was no statistical significance in without complication in patients with essential hypertension(P<0.05);(3)In patients with essential hypertension,the higher SBP and poor drug compliance was associated with Higher BPV(P<0.05);(4)Furthermore,hypertension complications was associated with age and BMI(P<0.05);(5)The poor drug compliance in patients with essential hypertension was associated with Higher BPV(P<0.05);(6)During the follw-up,two patients with essential hypertension from community health service center and the blood pressure variability is higher than the median CV,but because the sample number of endpoint events is less,it is not enough to carry on the statistical analysis,and can not prove that the blood pressure variability is related to the end point events.It needs to be bigger sample,and follow-up for longer to further confirm.Conclusion: The mean SBP level of patients with essential hypertension in community has an increasing trend with the increase of mean systolic blood pressure.The drug compliance of patients with essential hypertension in community is an important factor affecting BPV.In the future,when we work in community,for people with high blood pressure,we must not only focus on blood pressure average,but also to pay attention to the BPV.
