Objective To explore the the relationship between P-glycoprotein(P-gp)expressi on and aspirin resistance(AR)in patients with ischemic stroke,the the relationship between AR patients who with ischemic stroke and polymorphism of ABCB1 gene(multidrug resistance gene),and the relationship between P-gp expression and poly morphism of ABCB1 gene.Methods 265 patients with cerebral infarction who were hospitalized in Departme nt of Neurology,affiliated Hospital of Qingdao University were selected as study subjects during the period from June 2016 to September 2017.All the patients in t he group met the diagnostic criteria of Chinese guidelines for the diagnosis and tr eatment of acute ischemic stroke.,and the diagnosis was confirmed by cranial MRI or CT examination.According to the results of thrombelastogram(TEG)and platel et aggregation,patients were divided into two groups : AR 93 cases and aspirin s ensitive(AS)172 cases.The expression of P-gp in lymphocyte was detected by Western blotting technique.Polymerase chain reaction restriction fragment length p olymorphism(PCR-RFLP)and direct sequencing were used to detect the polymor phism of G2677T/A,C1236 T and C3435 T in ABCB1 gene.Statistical Analysis wi th SPSS22.0.Results The expression of P-gp in lymphocyte population of ischemic stroke was significantly different between AR and AS,in AR patients was higher than that of AS patients(P<0.5).The distribution frequency of ABCB1 gene was in accor dance with the Hardy-Weinberg equilibrium law.The frequency of CT genotypes at C3435 T locus in AR group was higher than that in as group,and the difference between the two groups was statistically significant.T allele frequency of C3435 T i n AR group and AS group were 51.1% and 48.3%,the difference was statistically significant(OR=1.765,P=0.018).For the other two loci,C1236 T and G2677T/A,t here was no significant difference in allele frequency and genotype between AR g roup and as group(P > 0.05).In AR patients,the correlation between the expressi on level of P-gp in lymphocyte and C3435T(CT>TT>CC,P<0.05),There was no statistical difference in AS patients.There was no significant difference in P-gp expression between C1236 T and G2677 T / A genotypes.(P>0.05).Conclusion The occurrence of AR in ischemic stroke patients may be related to the overexpression of P-gp and the increase of P-gp expression may be related to the C3435 T genotype.Single nucleotide polymorphisms of ABCB1 gene C3435 T ar e associated with AR in patients with ischemic stroke.
