Abstract/Summary:
Objective:Objective to investigate the changes and significance of atypical lymphocytes and lymphocyte subsets in children with infectious mononucleosis.Methods:41 cases of infectious mononucleosis in the Affiliated Hospital of Qiingdao University,December 2016-2017 and December were selected as the experimental group,and 60 healthy children were selected as the control group at the same time.All the subjects were taken fasting venous blood.After extracting DNA,the number of EBV in whole blood was measured by fluorescence quantitative PCR,and the ratio of the hetero lymphocyte and the blood cell count were measured.The lymphocyte subsets(the proportion of CD3+T lymphocyte,the proportion of CD8+T lymphocyte in CD4+T lymphocyte proportion,the proportion of CD8+T lymphocyte in CD4+T)were measured by flow cytometry.CD56+ CD16+ ratio,etc.,to compare the differences between the two groups,and to analyze the correlation between atypical lymphocytes and lymphocyte subsets.SPSS19.0 statistics software package is used for data processing.The measurement data were expressed by mean number of standard deviation(s).T test was used to compare the data between normal distribution and homogeneity of variance,and non parameter test was used for the non conforming.Counting data were expressed by percentage(%);the count data between groups were tested by x2 test;correlation analysis was conducted by Spearman correlation test.The difference between P ≤ 0.05 was statistically significant.Results:(1)there was no significant difference in sex,age,height and weight between the two groups(P > 0.05),but the temperature of the children in the experimental group was significantly higher than that in the control group(P < 0.05).(2)the ratio of the heterotype lymphocyte in the experimental group was(8.25 + 2.35)%,while the ratio of the heteromorphic lymphocyte in the control group was(0.82 + 0.25)%,and the difference was statistically significant(P < 0.05).(3)the proportion of CD3+T lymphocyte and CD8+T lymphocyte in the experimental group was significantly higher han that in the control group(P < 0.05),while the proportion of CD4+T lymphocyte,CD4+/ CD8+ and CD56+ CD16+ were significantly lower than those of the control group(P < 0.05).(4)the proportion of heterocyst in infectious mononucleosis was positively correlated with the total number of r=0.524(P=0.015)and the count of lymphocyte(r=0.661,P=0.01).(5)heteromorphic lymphocytes were positively correlated with CD3+T lymphocyte count(r=0.497,P=0.036),CD8+T lymphocyte count(r=0.517,P=0.028)and CD56+ CD16+ count(r=0.511,P=0.030),and negatively correlated with CD4+/CD8+(r=-0.527,);heteromorphic lymphatic finer The cells were positively correlated with the total number of T lymphocytes(r=0.627,P=0.009)and activated T cells(r=0.612,P=0.007),and negatively correlated with the regulation of T cells(r=-0.578,P=0.015),and was not related to the activation of B cells(r=-0.249,P=0.319).(6)there was no correlation between EBV-DNA titer and R=0.190(P=0.239)in the experimental group.Conclusion:(1)The ratio of heterotypic lymphocytes in children infected with infectious mononucleosis was significantly higher than that in normal children.(2)There was obvious T lymphocyte immune disorder in EBV infected children with infectious mononucleosis,the increase of CD8+T lymphocyte,the decrease of CD4+T lymphocyte proportion and the increase of CD3+T lymphocyte proportion.(3)Proportion of infected mononucleosis heterocyst was positively correlated with the total number of peripheral white blood cells,the total number of lymphocytes,the count of T lymphocyte,and the activation of T cells,and may be related to the count of CD3+T lymphocyte,the count of CD8+T lymphocyte and the CD4+ /CD8+.(4)The number of atypical lymphocytes in peripheral blood of children with infectious mononucleosis can indicate the extent of immune activation.
