Objective Objective to investigate the relationship between serum levels of 4-hydroxyl-nonenal?4-HNE?and the grading of pulmonary function in patients with acute exacerbation of chronic obstructive pulmonary disease?AECOPD?,and to analyze the clinical diagnostic value of 4-HNEin patients with AECOPD.Methods The total of 60 patients with AECOPD were collected in our study.All cases were inpatients during December of 2016 to December of 2017 from Department of Emergency of the Laoshan district of The affiliated Hospital of Qingdao University.The levels of 4-HNE was measured by enzyme-linked immuno sorbent assay?ELISA?.The levels of GOLD classify was measured bypulmonary function test.The 60 patients were divided into group AGOLD level 2?n=20?,group BGOLD level 3?n=20?,group CGOLD level 4?n=20?,according to FEV1percentage of predicted value?FEV1%pre?.The clinical data of three groups of patients were collected at the same time.Theealthy physical examination as a control,4-HNE was also measured in this people.To compare the difference of each 4-HNE,by SPSS21.0 application for statistical analysis.Results According to the GOLD classify;with the increase of FEV1%pre,4-HNEhas a tendency to declinegradually?group C>group B>group A?.There is statistically significant?P<0.05?in group A and group C.The average levels of 4-HNE in A,B,C groups and control group by single factor variance analysis showed that F=77.166,P<0.001.The correlation analysis of 4-HNE and FEV1%pre showed that r=-0.749,P<0.05.Univariate analysis showed that gender,paCO2,4-HNE and HCO3-could affect pulmonary function grading.;multivariate Logistic regression analysis showed that 4-HNE and HCO3-were independent risk factors of pulmonary function grading.With A,B,C three groups as the research object,4-HNEas a diagnostic classification,the ROC curve showed that the4-HNE area under curve?AUC?was 0.871,the critical value of 17.25 mg/L,sensitivity of 80%,specificity of 80%.ConclusionThe serum 4-HNE in AECOPD patients was negative correlation with FEV1%pre,4-HNE was useful in risk stratification and assessment of the severity of AECOPD patients.
