The Current Treatment Of Psoriasis Vulgaris In Qingdao:A Retrospective Study

Objective: To review the current treatment status of psoriasis vulgaris in Qingdao by examining medical care-seeking behavior,health economics(efficacy/cost),Dermatology Life Quality Index(DLIQ),incidence of post-treatment adverse events,and physician prescribing behavior to provide insights into a standardized clinical treatment for psoriasis vulgaris.Methods: Three hundred twenty psoriasis vulgaris patients and 60 dermatologists affiliated with mainstream or non-mainstream hospitals in Qingdao were included in this study.Patient demographics were collected through a self-completed form,and the collected data were statistically analyzed and partially compared with the findings from the 1998 survey.Results: 1.The average age of the 320 patients with psoriasis vulgaris was(40.62 +14.067),and the male and female ratio was 1.162:1,the mean age of the male and female was not significantly different(p >0.05).The average age of onset of the patient was(28.12 + 13.367),and there was no significant difference in the age of onset of male and female(p > 0.05).2.84(26.25%)patients have a family history,73 patients with family history from type I psoriasis(early hairstyle,starting age < 40),11 patients with family history from type II psoriasis(late onset,starting age 40 or higher),there was significantly different(28.97%vs16.18%,p<0.05).The infection accounted for 48.98% of the induced factors,26.53% of the mental factors and 24.49% of the fatigue.3.The proportion of patients only seeking treatment from mainstream hospitals was significantly higher than in 1998(58.13% vs.45.59%,p<0.05),but no difference was observed in the 3-year improvement rate between 2016 and 1998(25.63% vs.27.94%,p>0.05).4.The mean cost of treatment for patients in mainstream hospitals was 403.68yuan(0-5,000 yuan),and the median cost was 300 yuan.The mean cost of treatment in non-mainstream hospitals was 1692 yuan(10-10,000 yuan),and the median cost was1600 yuan.Mann-Whitney U test showed that the latter was significantly lower than the former(p<0.001).5.Most dermatologists(90%)in mainstream hospitals could provide customized treatment plans based on the type of skin lesion and severity and duration of disease.Therefore,treatments from mainstream hospitals were significantly more standardized,effective and safe than those prescribed by non-mainstream hospitals(p<0.05).Moreover,they were significantly lower than that in 1998(p<0.05).6.The DLIQ score was significantly lower among patients as a function of the cost of the course of treatment(<300 Yuan vs.300-600 Yuan and ?600 Yuan,p<0.05).Similarly,the DLIQ score of patients who paid 300–600 Yuan for treatment was significantly lower than patients who paid ?600 Yuan(p<0.05).The DLIQ score of mild psoriasis patients was lower than that of moderate and severe psoriasis patients,while the DLIQ score of moderate psoriasis patients was lower than that of severe psoriasis patients(P < 0.01).The cost treatment,the severity of the disease and the DLIQ score were positively correlated(P < 0.01,r=0.391,0.418),and the cost of treatment was still positively correlated with the DLIQ score under the control of the influencing factors of the condition of the disease(P < 0.01,r=0.251).Conclusion: The population distribution of psoriasis vulgaris patients in Qingdao was consistent with previous epidemiological studies.Medical care-seeking behavior of psoriasis vulgaris patients in Qingdao is much better than medical care-seeking behavior19 years ago,but the long-term efficacy was not significantly improved.The mainstream hospital physicians adopt the concept of systemic treatment,principles of health economics,personalized treatment and are more economical,standardized,and effective compared to treatments provided by physicians affiliated with non-mainstream hospitals.Dermatologists affiliated with mainstream hospitals should promote the popularization of standardized treatment for psoriasis vulgaris and increase patient knowledge of the disease to increase awareness of the current treatment status of psoriasis vulgaris to control the development of the disease or to prevent the recurrence of the disease.
