Significance Of HR-HV Detection In The Triage Of ASCUS Based On Cervical Fluid Cytology Test

Objective Cervical canceris is one of the common tumors in female genital duct,it is widely believed that the persistent infection of HR-HPV is the key factor of cervical high-level intraepithelial neoplasia and cervical cancer.Effective screening can find people at high risk and prevent the occurrence of cervical cancer by taking appropriate interventions.TCT has its own advantages such as high specificity and sensitivity.But at the same time strong subjective judgment and experience dependence are the disadvantages,and as cervical cancer screening population are mainly healthy women,follow-up tracking is difficult.In this study we intend to investigate the application of high-risk human papillomavirus(HR-HPV)detection in diversion of cervical atypical squamous cell of undetermined significance(ASCUS)patient.Methods 1862 women were detected by thin layer cytologic test(TCT)technique,in which women diagnosed as ASCUS were all examined by HR-HPV and colposcopic pathology histopathological biopsy.Results There were 196 patients diagnosed with ASCUS in the 1862 women and the positive rate was 10.5%.The positive rate of HR-HPV in women who with ASCUS was 31.1%,and it was higher in≥35-year-old group(70%)than that in <35-year-old group(14%),and the difference was significant(X2=61,P<0.01).The infection of HR-HPV was also closely correlated to the high-level cervical intraepithelial neoplasia(X2=42.4,P<0.01).Further more,the correlation was existed both in≥35-year-old group and <35-year-old group(X2=4.8,P<0.05;P=0.0181).Both of the infective rate of HR-HPV and the positive rate of pathologic diagnosis in≥35-year-old group were higher than those in<35-year-old group(X2=65.52,39.01;P<0.01,respectively).There was differrence in the two age groups both of HR-HPV infection rate and positive pathological diagnosis(X2=4.8,P<0.05;P=0.0181,respectively).Conclusion Colposcopy directed biopsy should be suggested for ASCUS cases with positive HR-HPV for early detection and treatment of the corresponding lesions.ASCUS cases with negative HR-HPV is recommended for regular TCT and follow-up.
