Objective:With the development of cervical cancer screening technology, the incidence of cervical cancer in developed countries has declined significantly, but in developing countries cervical cancer is still the second most common malignant tumor of women.jeopardizeing the health of women seriously due to its high morbidity and mortality. As is known to all, the occurrence of cervical cancer is influenced by many factors. It has been proved that persistent infection of high-risk HPV is the main cause of cervical cancer and cervical intraepithelial neoplasia. But the vast majority of CIN can be self-limiting.Therefore, in addition to HPV infection, some other co-factors can affect the development of cervical lesions. The latest research shows that the deficiency of floate has been identified to be associated with the development of cervical cancer but no one set forth the specific pathogenesis up to now. So the purpose of this study is to detect the the level of serum floate of participants, discuss its role in the development of cervical lesions, and discovery the potential biological indicators for CIN lesion, and provide novel findings for the prevention and treatment of cervical cancer. Methods:Individuals for health physical examination at maternal and child care service centre of jiexiu, shanxi of married women 20-65 years for the survey, from June 2014 to December 2014 were enrolled. TCT was performed on all of them after filling in the questionnaire, Those with abnormal cervical glandular cells were excluded. HPV-DNA by using PCR and Membrane hybridization technique for those who were diagnosed asASCUS and above, take cervical tissues by using electron colposcope. Those who were diagnosed as LSIL(288) and HSIL(82) by biopsy were recruited, take others as control group(362). Collect venous blood of all the participants with empty stomach, detect the level of serum folate by chemiluminescence.Results:1.There were no significant differences in age, smoking history, age at menarche, menopause status, age at first intercourse among the control,LSIL and HSIL groups, however, there were significant differences in hr-HPV infection and delivery times.2.In the control, LSIL and HSIL groups, the serum folate levels were 18.86 ±10.12,17.41±9.00,15.75±6.58nmol/L(median ± interquartile number), respectively. The level of serum folate among the three groups was significantly different according to the K–W test(2c= 14.72, P=0.001). women with LSIL and HSIL had a significantly lower folate status than those with normal cervical histology(P<0.001), and women with LSIL had a significantly higher serum folate level than HSIL(P<0.001).3.In multinomial logistic regression analysis of relevant factors of CIN, referring to the control group,In the case of other variables constant, comparing with normal people,HPV negative resulting in LSIL was 0.706 times as those HPV positive, delivering more than 3 times resulting in LSIL was 1.620 times as those who deliveried less than 3times;referring to the control group,In the case of other variables constant, comparing with normal people, HPV negative resulting in HSIL was 0.088 times as those HPV postive, delivering more than 3 times resulting in HSIL was 1.440 times as those who deliveried less than 3 times;serum folate elevating one unit resulting in LSIL was 0.928 times as those remain.4.Interaction between serum folate and hr-HPV infection and delivery times.1compare to HPV negative high level of serum folate group, the risk of HPV postive low level of serum folate group was 3.848 times, whele high level of serum folate group was 1.819 times. 2 compare to more than 3 times delivery high level of serum folategroup, the risk of more than 3 times low level of serum folate group was 2.680 times, whele high level of serum folate group was 1.599 times. Conclusion:Low folate status was involved in CIN,with the development of CIN,serum folate levels decreased, higher serum folate is a protective factor for CIN, low levels of serum folate may promote the development of CIN through collaboration with the relevant factors.
