| ObjectiveTo establish the checklist of ICU bedside handover,and to explore the application effect of the checklist in ICU junior nurses beside handover,to improve the quality of the beside handover,and to provide a new mode of handover for the bedside handover.MethodsThis study is divided into two parts.The first part:establish a list research group.The checklist of ICU bedside handover was established based on the literature reviewing and clinical nursing practicing,and the final draft was established by organizing the expert group meeting.The second part:A convenience sampling method was used to select junior nurses who met the inclusion criteria in the intensive care unit of grade three level A hospital in Qingdao.Prior to the use of the ICU bedside handover list:From March to April in 2017,the ICU junior nurses in accordance with the standards adopted the regular bedside handover mode for handover.After the use of the ICU bedside handover list:from July to June in 2017,after the training of the ICU junior nurses,the handover were carried out by the checklist of ICU bedside handover.The number of nurses who did not carry out hand hygiene,the number of the cases of indwelling needle plugging,the number of deep venous catheterization,the number of artificial airway air bag pressure,the number of head elevating non standard cases,the number of cases of catheter removal,and the evaluation of the effect of the handover class and the time of each patient's transfer.Results(1)Nursing quality is implemented before and after using the listAfter the statistical analysis,after making use of ICU bedside handover checklist,the number of nurses did not carry out hand hygiene(2.12%)was significantly lower than that before the use of ICU bedside handover list(2.12%),and the difference was statistically significant(c~2=4.731,P=0.030),the number of cases of catheter plugging(4.25%)was obviously lower than the number of cases(4.25%).and the difference was statistically significant(c~2=4.372,P=0.037)before the ICU bedside transfer class(c~2=4.372,P=0.037);the number of cases of deep venous catheterization(5.17%)was significantly lower than that before the use of the ICU bed(17.30%),and the difference was statistically significant(c~2=4.154,P=0.042),and the number of non standard cases of artificial airway pressure(6.25%)was significantly lower than that of the group(17.30%).The difference was statistically significant(c~2=4.225,P=0.040)before the ICU bedside handover class(c~2=4.225,P=0.040),and the number of bedside elevation non standard cases(10.63%)was significantly lower than that before the use list(29.78%),and the difference was statistically significant(c~2=10.686,P=0.001),but there was no significant difference in the number of cases of catheter removal(c~2=10.686,P>0.05).(2)Effect evaluation of nurse shift work before and after using the listAfter using the checklist,the effect of the junior nurses on handover is obviously higher than that before using the list.The results showed that the average score was 59.03±3.85 before using the list,and the average score was 71.84±7.59 after using the list,and the difference of two scores was statistically significant(t=14.191,P<0.001).(3)Average patient handover time before and after using the listThe average handover time for each patient before using the list was 10.19±1.87minutes,while after the use of the list the average shift time of each patient was 10.56±1.77 minutes.There is no significant differences in the average handover time(t=1.401,P=0.163).ConclusionThe checklist of ICU bedside handover can effectively improve quality of handover.It standardized the content and form of the handover.It improved the junior nurses'awareness rate of patients'condition and the predicted ability of unsafe factors.It also improved the quality of nursing and reduced the risk of nursing.It improved the evaluation of the handover effect from the junior nurses.It ensured the quality of handover,and didn't increase the handover time of nurses. |
