Objective: The purpose of this study is to investigate the Montreal cognitive assessment and its application feasibility in Chongqing city community elderly people with mild cognitive impairment screening value,analysis of the Chongqing city community elderly people with mild cognitive impairment screening sensitivity,specificity and Youden index,determine the optimal cut-off in Chongqing city community screening work value.At the same time,to explore whether it has universal applicability in the screening of mild cognitive impairment among the elderly in different educational level in the community,so as to lay a theoretical foundation for the future community health service center to achieve the MCI screening of the elderly.Methods: From September 2016 to December 2017,the community was randomly selected from the city of Chongqing to investigate the elderly population.The research tools were self-made personal information scale,Montreal cognitive assessment text(MoCA),mini mental state examination(MMSE),Global Deterioration Scale(GDS),activity of daily living scale(ADL),and Clinical Dementia Rating(CDR).The MCI were diagnosed by neurosurgeon.615 subjects of data collected will be overall analysis,to obtain the scale sensitivity and specificity in screening of MCI in the community of Chongqing city and the Youden index,using the maximum Youden index to determine the optimal cut-off value of scale screening,to determine its applicability in the community elderly population screening for the MCI.Due to the fact that the consistency test did not achieve complete agreement,we sought to find out the shortcomings of the scale screening.Then we screened 73 MCI patients' MoCA scale information for different education levels,and evaluated the impact of education years on MoCA scores,and comprehensively evaluated the application value of MoCA in communities.Results: 24 is the best suitable cut-off value in screening of MCI in Chongqing community elderly.The sensitivity was 93.15%,specificity was 92.74%,Youden index was 0.8589.The Kappa value is 0.715 and has good consistency.The three differences of visual space and executive ability,language and delayed memory were not statistically significant between the three groups based on the number of years of education.There were statistical significance between the groups of naming,attention,abstraction,orientation and total score.The primary school group and the middle school group?the high school group were statistically significant in naming,noticing,abstract,orienting and total score.There was no significantdifference between the items of the junior high school group and the high school group(P < 0.05).Conclusion: The Montreal cognitive assessment text is feasible for the mild cognitive impairment screening tool for the elderly in the community,and 24 points are recommended as the cut-off value of MCI screening for the elderly in Chongqing community.MoCA text has significant difference in naming,attention,abstraction,orientation and total score in evaluating MCI patients of different educational level.The applicability of low education level to scale is lower than that of high education level.People with low education need more guidance.
