Objective:Study of blood loss and safety on repeated intravenous drip of tranexamic acid on unilateral intertrochanteric fracture treated with PFNA.Method:Select 60 patients with unilateral intertrochanteric fracture in the elder were treated with PFNA,they were randomly divided into two groups according to sex and age?there were thirty cases in each group.Group A:Single intravenous infusion group(twenty minutes before surgery to give fifteen mg/kg weight tranexamic acid infusion)and Group B:Repeated intravenous infusion group(twenty minutes before surgery to give fifteen mg/kg weight tranexamic acid infusion and seven hours after treated with one g intravenous drip of tranexamic acid).The preoperative data,cooperative data and complications were compared between the two groups.The blood loss and safety of the two groups were evaluated by routine blood test and double lower extremity ultrasound in the postoperative.Result:Comparison of preoperative data between the two groups,the differences were not statistically significant(P>0.05),Comparable.The total blood loss,occult blood loss,postoperative blood transfusion rate,the average blood transfusion volume,the maximum decrease of postoperative hemoglobin,the postoperative drainage volume and postoperative hospital stay were compared between the two groups,the differences were statistically significant(P<0.05).Comparison of operation time between two groups,the differences were not statistically significant(P>0.05).No deep vein thrombosis,pulmonary embolism and wound infection occurred in all the two groups.The incidence rates of inter muscular vein thrombosis,effusion,hematology and fat liquefaction were compared between the two groups,the differences were not statistically significant(P>0.05).Conclusion:Compared with the single intravenous drip of tranexamic acid,repeated intravenous infusion is more effective in controlling cooperative blood loss,also it does not increase the risk of deep vein thrombosis,Safe and effective,it is worth popularizing.
