Electrical cardioversion(ECV)is an important treatment of tachyarrhythmia,which possesses many advantages such as high success rate,effect rapidly,easy-to-operate and safe,etc.It is the preferred method for clinical treatment of fatal tachyarrhythmia and an another choice for the patients failing of drug treatment.However,there are still a few patients failing to be cardioverted.Because of the small number of cases,there is a lack of a randomized,double-blind,case-control study of the reasons for the failure of electrical cardioversion for tachyarrhythmia,such a clinically important problem cannot be solved by meta-analysis.This article reviews the basic research and case reports related to the failure of electrical cardioversion and discusses the reasons that may lead to the failure of electrical cardioversion.Finally,it was found that the energy size and output form,electrode plate size and location,transthoracic impedance and other technical factors are important reasons affecting the failure of electrical cardioversion.Patient-self factors such as the patient's baseline conditions,presence of underlying lesions and extent of the lesion,duration of arrhythmia,and combined diseases,etc.may also lead to the failure of electrical cardioversion.
