Objective:Coal worker's pneumoconiosis had different degrees of pulmonary function damage with the development of the disease.There was no effective way to cure this disease.The treatment methods both in china and abroad were limited to symptomatic and supportive clinical treatment,in order to relieve patients'pain and improve their quality of life.This study was based on home oxygen therapy for patients of coal worker's pneumoconiosis,pulmonary function,blood gas and BODE indexes were detected before oxygen therapy?0 month?and oxygen therapy for 6 months and 12 months,The correlations between these factors were also explored,to investigate the effects of home oxygen therapy on pulmonary function,blood gas index and quality of life in patients of coal worker's pneumoconiosis.Methods:159 patients with coal worker's pneumoconiosis were enrolled in this study,36 patients without oxygen were collected as control group.123patients with coal pneumoconiosis were treated with home oxygen therapy?oxygen time:15 hours/day,oxygen concentration:2 liters per minute?and followed up for a long period of 12 months.On this basis,the pulmonary function,arterial blood gas and BODE index in 0,6 and 12 months were measured,and the correlations between these indexes were also analyzed.All the data were analyzed with SPSS 22.0 software for statistical analysis,two means were compared with unpaired t test,F test?analysis of variance?was used in group comparison among more than two groups,LSD-t test was used in pairwise comparisons of many means;Numeration data were statistically analyzed with?2 test,correlation of measurement data were analyzed by Pearson correlation test and the correlation coefficient?r?was also calculated.All hypothesis tests were performed using a two-sided test.P<0.05 was considered as statistically significant.Result:1 With the increase of oxygen therapy time in patients of coal worker's pneumoconiosis,there were no significant decline in the VC,FEV1/Pre,FEV1/FVC,DLCO,PEF indicators?P>0.05?;2 In the 0,6 and 12 month,the arterial oxygen pressure in the observation group was significantly lower than that in the control group,and the partial pressure of carbon dioxide in the observation group was significantly higher than that in the control group?P<0.05?.With the increase of oxygen therapy time in the observation group,the arterial oxygen partial pressure gradually increased and the partial pressure of carbon dioxide in the arterial blood gradually decreased?P<0.05?;3 After oxygen therapy for 6 months in the observation group,the FEV1/Pre did not show a decreasing trend,BMI and 6MWT indexes gradually increased,and MMRC and BODE indexes decreased,when compared with 0month.After 12 months of oxygen therapy,the indicators of patients improved more significantly?P<0.05?.4 The correlation analysis of pulmonary function,arterial blood gas analysis and BODE indexes in the observation group were showed as follows:?1?In the observation group,VC,FEV1/Pre,FEV1/FVC?PEF and DLCO were positively correlated with PaO2?P<0.05?;VC,FEV1/Pre,FEV1/FVC,PEF were negatively correlated with PaCO2?P<0.05?;There was no correlation between DLCO and PaCO2?P>0.05?.?2?In the observation group,FEV1/Pre was positively correlated with pulmonary function indexes of VC,FEV1/Pre,FEV1/FVC,PEF?P<0.05?;6MWT was positively correlated withVC?FEV1/FVC,PEF,DLCO?P<0.05?;BODEwerenegativelycorrelatedwithVC,FEV1/PreandFEV1/FVC?P<0.05?;There was no correlation between the other indexes?P>0.05?.?3?FEV1/Pre,BMI?6MWT and BODE in observation group were positively correlated with PaO2;FEV1/Pre?BMI?6MWT and MMRCwere negatively correlated with PaCO2?P<0.05?;There was no correlation between the other indexes?P>0.05?.Conclusion:1 Home oxygen therapy could delay the development of respiratory difficulties in patients.Pulmonary function could sensitively reflect the progress of coal worker's pneumoconiosis,and it could provide early warning for coal worker's pneumoconiosis diagnosis;2 Home oxygen therapy had a good effect for patients of coal worker's pneumoconiosis,which could improve the patient's cardiopulmonary function and the degree of hypoxia directly by increasing the blood oxygen pressure.Secondly,it could accelerate the discharge of carbon dioxide,and reduce the risk of acidosis caused by excess carbon dioxide in the body by reducing the partial pressure of arterial carbon dioxide;3 After oxygen inhalation,no significant deterioration in the symptoms of dyspnea were observed,exercise endurance was improved,and physical strength of patients was recovered to a certain extent,thus improving the quality of life.The longer the time of oxygen therapy in patients of coal worker's pneumoconiosis,the more obvious the improvement of all the indexes;4 Pulmonary function indexes,arterial blood gas and BODE indexes had good correlations.According to the data of pulmonary function test,arterial blood gas index and BODE index of patients could be extrapolated.Pulmonary function indexes,arterial blood gas and BODE indexes could reflect each other,and there was a certain reference to each other.
