Objective: To explore the relationship between delayed hydrocephalus and different grades and treatment modalities in patients with spontaneous intraventricular hemorrhage,and study the mechanism of delayed hydrocephalus and the effect of different treatments on its incidence.Methods: 156 patients in October 2015-October 2017 in Hebei North University first hospital affiliated hospital neurosurgery intraventricular hemorrhage diagnosed as cerebral hemorrhage by brain CT/MR examination were enrolled in this study.According to the different treatment methods,the patients were divided into 75 cases of internal medicine treatment group,including 21 cases of delayed hydrocephalus and 81 cases of surgical treatment group,of which 6 cases had delayed hydrocephalus.Based on the spontaneous intraventricular hemorrhage class grading methods are divided into ?-?(0 to 5 divided into ? level,level 6-10 divided into ?,11 to 15 are rated ?,16-20 divided into ? level),grade ? 28 cases,? 53,? level58 cases,? level of 17 cases,which occur delayed hemorrhage,a total of 27 patients with hydrocephalus,? level 1,level ? 8 cases,? level 13 cases,?level 5 cases.In 27 cases of delayed hydrocephalus after ventricular hemorrhage,1 cases were treated with grade I,0 cases were treated with surgical treatment,7 cases of second grade internal medicine treatment,1cases of surgical treatment,10 cases of grade III internal medicine treatment,3 cases of surgical treatment,3 cases of grade IV internal medicine and 2cases of surgical treatment.Objective to analyze the relationship between delayed spontaneous hydrocephalus and different grades and treatment methods of spontaneous intraventricular hemorrhage.Results:1.The incidence of delayed hydrocephalus in the internal medicine treatment group was 28%,and the incidence of surgical treatment group was7.41%.The difference was statistically significant(P=0.003).2.Spontaneous intraventricular hemorrhage class ? late-onset incidence of hydrocephalus by 3.57%,? level incidence by 15.09%,? levels rate22.41%,? incidence of 29.41%,the higher the grade,the incidence of late-onset hydrocephalus is higher.The incidence of late-onset hydrocephalus was 11.76%,14.29% in the 35-49 years,17.78% in the 50-60 years,20.33%in >60 years,and the higher the age,the higher the incidence of late-onset hydrocephalus.3.The incidence of delayed hydrocephalus in the treatment group of grade II patients was 28%,the rate of the surgical treatment group was 3.75%,the incidence of delayed hydrocephalus in the internal medicine treatment group of grade ? was 37.04%,the rate of the surgical treatment group was9.68%,and the incidence of delayed hydrocephalus was obviously compared with the internal medicine treatment in the patients with grade II and III.The difference was statistically significant(P=0.037,P=0.045,respectively).Conclusions:1.The incidence of delayed hydrocephalus in ventricular hemorrhage after surgical treatment is lower than that of internal medicine.In patients with spontaneous ventricular hemorrhage grade II and grade III,the incidence of delayed hydrocephalus is significantly reduced by surgical treatment,so active surgical treatment is more beneficial to such patients.2.A comprehensive assessment of the patients with cerebral hemorrhage,taking individualized treatment measures and active surgical intervention can obviously reduce the incidence of delayed hydrocephalus and improve the efficiency and survival rate of the patients with cerebral hemorrhage.
