Objective: To evaluate the possible role of M—type phospholipase A2 receptor(PLA2R)antibody in the estimation of the clinical and therapeutic response of idiopathic membranous nephropathy(IMN)Methods: 250 IMN patients diagnosed by renal biopsy in Nephrology Department of Shengjing Hospital affiliated to China Medical University from Jan 2014 to Dec 2017 were enrolled in the study.All the patients were determined the level of PLA2 R antibody in baseline.According to the PLA2 R antibody level,patients were divided into two groups,negative group(73 cases)and positive group(177 cases).The difference of biochemical indexes in two groups was analyzed.Among them,154 cases(46 cases in negative group and 108 cases in positive group)were followed up for at least 6months.Therapeutic response was observed in order to evaluate the possible role of PLA2 R antibody in predicting the clinical prognosis.Results: There were significant difference in the 24 hour urinary protein(P=0.010),urine sediment red blood cell count(P=0.028),serum albumin(P=0.002)and high-density lipoprotein(P=0.045)between two groups.The therapeutic response was divided into effective and ineffective.The total effective rate of the negative group was higher than that of the positive group,and the remissionoccurred in earlier stage in negative group(P<0.05)Conclusions: The determination of serum PLA2 R antibody level can preliminarily evaluate the patient's clinical condition and help to judge the prognosis,thus it has important clinical significance.
