The Diagnostic Value Of Detecting The Feature Of Plaque Qualitatively By CTA For Predicting Myocardial Ischemia Detecting By MPI

Objective: To study the diagnostic value of detecting the feature of plaque qualitatively and the degree of stenosis by CTA for predicting myocardial ischemia in the patients with myocardial ischemia detecting by SPECT.Methods: The 60 patients had a examination of CTA and SPECT among 7 days,we collected clinnical features of patients(Gender,Hypertension,Hyperlipidemia,Smoking,Diabetes),detected the degree of stenosis in coronary artery and the feature of plaque qualitatively,including “ low attenuation ” plaque(LAP),“ positive ” vessel romodeling(PR),spotty calcificiation(SC)and napkin-ring sign.We analyed the images of SPECT by using 17 segments of myocardial system of myocardial system.The results were divided into the myocardial ischemic positive group and the myocardial ischemic negative group.The comparsion between groups using independent sample chi-square tests for classification variables,and the comparsion of diagnostic performance of myocardial ischemic in significant stenosis(? 70%),high-risk plaque and the two-combined by using ROC curve.Results: n the clinical features of the patients,the difference included Wemen and Hyperlipids in the comparison between myocardial ischemia and non-myocardial ischemia is statistically significant(P < 0.05).IN the qualitative characteristics of coronary atery.The difference included significant stenosis(?50%)?“positive” vessel romodeling(PR)?napkin-ring sign and three or more characteristics of high risk plaque between myocardial ischemia and non-myocardial ischemic groups is statistically significant(P < 0.05).The sensitivity,specificity and accuracy of predicting myocardial ischemia by significant stenosis are 75.4%,50.9%and59.5%.The sensitivity,specificity and accuracy of predicting myocardial ischemia by high risk plaque are 78.7%,34.8%and 50.3%.The sensitivity,specificity and accuracy of predicting myocardial ischemia by the two combined are 73.8%,75.0% and 74.6%.The area under the ROC curve of predicting myocardial ischemia by significant stenosis combined with high risk plaque is0.728(95%CI:0.649~0.807).Conclusion: DWI and Predicting myocardial ischemia by significant stenosis combinedwith high risk plaque is more accurate,and the combination of the two characteristis could make a better diagnosis of myocardial ischemia.
