Study On The Effect Of Comorbidities In Chronic Obstructive Pulmonary Disease On CAT Score

Objective: To investigate whether the comorbidities of chronic obstructive pulmonary disease(COPD)affects CAT score and guide the classification of grading and clinical treatment in stable COPD.Methods: AECOPD patients hospitalized in the First Affiliated Hospital of China Medical University and Shenyang Medical College Affiliated Central Hospital Respiratory and Critical Care Unit were selected from April 1,2016 to December 1,2017.The basic demographic data and past history of patients were recorded on the day of admission.The CAT score,m MRC score and number of exacerbations(number of acute exacerbations in the past year)were recorded when the patients were in stable condition.Results: A total of 95 AECOPD patients were collected in this study,male(60%)and female(40%).In all patients,10.5% had no comorbidity,14.7% had one comorbidity,35.7% had two comorbidities,17.9% had three comorbidities,11.6% had four or more comorbidities.The top three comorbidities were hypertension(42.1%),coronary heart disease(38.9%)and cardiac dysfunction(29.4%),while osteoporosis and anxiety /depression were only 2 cases and 1 case.The presence or absence of co-morbidities in the CAT score,cardiac dysfunction and Gastroesophageal reflux disease were statistically significant(P less than 0.05).Multiple stepwise regression analysis: multiple stepwise regression equation: CAT =14.125 + 4.092 *m MRC + 4.372 *Gastroesophageal reflux disease+1.098 *Age(years)+ 7.104 *Cardiac insufficiency.Gastroesophageal reflux disease and Cardiac dysfunction accumulation R2 are 0.026 and 0.072,respectively.Conclusions: COPD often has multiple diseases,the most common is cardiovascular disease,This study of Cardiac insufficiency,Gastroesophageal reflux disease on the CAT score to a certain extent,but due to the small sample size,need to be more centers,large sample further studies to confirm whether comorbid diseases affect the CAT score.COPD should be clinically important of comorbid diseases.
