Objectives To study the effects of vinyl chloride on blood,kidneys and liver in vinly chloride monomer-exposed workers and to understand the mechanism of the harmful effects of vinyl chloride on the body.To study the effects of vinyl chloride on serum hormone levels,oxidative damage indicators,and tumor marker levels in vinly chloride monomer-exposed workers,and to find reasonable and effective biomarkers of effect.Methods Two hundred and twenty-three workers exposed to VCM from Chlor alkali plant were recruited for this study,having a minimum of one year VCM exposure and also one hundred and forty-nine workers as control group have been chosen who matched for their age,sex,length of service and with no occupational history of VCM exposure with above group.The subjects were collected fasting venous blood and end-stage urine,blood,urine routine and liver function were examined in exposed and control groups;Serum hormones?PRL,Testo,Pro,LH,FSH,and E2?levels and serum tumor markers?CEA,AFP,TPSA,glycosyl antigens CA-199,CA72-4,CA-153,CA-125,FPSA,?-HCG,NSE,FPSA/TPSA?levels were measured using chemiluminescence immunoassay.The levels of urine creatinine,?2-microglobulin and thiodiglycolic acid were measured.And the activity of oxidative damage indicators?T-SOD,MDA,HYP,T-AOC,and LDH?in serum was measured.Results 1 A total of 372 people were investigated in this study,223 in the exposed group and 149 in the control group.There was no significant difference in gender,smoking,drinking,and urine?2-microglobulin between the two groups?P>0.05?.The level of thiodiglycolic acid in workers exposed to vinyl chloride significantly higher than that in the controls?P<0.001?.2 The average concentration of vinyl chloride in the chloralkali plant air is 7.66mg/m3.3 RBC,HGB,HCT,WBC,LYM%,MXD%,NEUT%,MPV,TBIL,IBIL,GGT,ALT,A/G and so on were significantly different between the exposed group and the control group?P<0.05?.4 The urinary glucose,urobilinogen,and urine specific gravity of the exposed group were significantly different from those of the control group?P<0.05?.5 The abnormal levels of PRL and LH levels in the exposed group were significantly different from those in the control group?P<0.05?.6 There was a correlation between E2 and thiodiglycolic acid in the exposed group?P<0.05?.7 Univariate analysis and multivariate analysis showed that exposure to vinyl chloride had no significant effect on serum hormones?P<0.05?.8 There were significant differences in LDH,T-SOD and MDA between the exposed group and the control group?P<0.05?.9 Univariate analysis showed that exposure to VCM had an effect on the activity of LDH,T-SOD,and MDA?P<0.05?.Multiple linear regression analysis showed the levels of LDH,T-SOD and MDA were affected by vinyl chloride,the activities of LDH and MDA of the exposed group increased,and the activity of T-SOD decreased.10 The abnormal rates of five indexes,such as CEA,AFP,CA72-4,NSE and FPSA/TPSA in the control group and the exposed group,were statistically significant compared with the control group?P<0.05?.11 Univariate analysis showed that exposure to VCM had an effect on AFP and NSE?P<0.05?.Multiple analysis showed the levels of NSE was affected by vinyl chloride?P<0.05?,the level of NSE of the exposed group is lower than that of the control group.Conclusions 1 Thiodiglycolic Acid can be used as an exposure biomarker for vinyl chloride.2 The occupational exposure to vinyl chloride has had influence on workers' blood system,kidneys and liver.3 This study found that the exposure to vinyl chloride can increase the abnormal rate of prolactin and luteinizing hormone in the workers' serum.Whether they can be used as biomarkers of effect of vinyl chloride needs to be further validated in large sample size populations.4 This study found that exposure to vinyl chloride can increase the activity of LDH and MDA,and decrease the activity of T-SOD.LDH,T-SOD and MDA can be used as effect biomarkers for vinyl chloride.5 Under the current concentration of vinyl chloride,vinyl chloride can reduce the level of NSE,and NSE may be biomarker of effect of vinyl chloride.
