Objectives Through evaluate the vaginal microecosystem and the detection of HPV before and after treatment of patients with cervical lesions,to discuss the correlation between vaginal microecological changes and the outcome of high-risk human papillomavirus(HR-HPV)before and after treatment with therapeutic methods.Methods 264 patients from the Affiliated Hospital of North China University of Science and Technology from March 2016 to December 2017 were collected.Vaginal secretions of patients were detected to vaginal microecosystem taking the quintuplet enzyme method.Cervical cell were detected to cervical liquid based cytology test using TBS diagnosis and HR-HPV using the Cobas 4800 meathod.Colposcopically directed biopsy and the cervical biopsy do the histopathological examination.Patients with HPV infections and Low-grade squamous intraepithelial lesion diagnosed by biopsy histopathology,obtain different therapeutic methods after fully communication and informed consent?There were 41 patients who did not use any drugs or surgery in the observation group,39 cases of using Baofukangshuan,79 cases of using anti-HPV biological dressing(JB01-BD),and 36 cases of using nocadia rubra cell wall skeleton for external use(Nr-CWS).A total of 69 patients undergoing cold knife cervical conization for the high squamous intraepithelial lesion diagnosed by biopsy histopathology.After 3 months,6 months and 12 months of treatment,HR-HPV typing,cytology test and vaginal microecological tests were performed again.Results 1 There was no significant difference in the ratio of vaginal vaginal pH ? 4.6 between the observation group and Baofukang group before and after treatment(P>0.05),in the others groups the difference was statistically significant(P<0.05).At the 6th and 12 th months after treatment,the observation group had the lowest ratio of pH ? 4.6,and the nocadia rubra cell wall skeleton for external use group had the highest ratio of pH ? 4.6.The difference was statistically significant(P<0.05).2 The proportion of vaginal lactobacillus in the observation group and Baofukang group before and after treatment had no significant difference(P>0.05).The proportion of lactobacillus in other groups increased compared with that before treatment(P<0.05),The proportion of vaginal lactobacillus in nocadia rubra cell wall skeleton for external use group was the highest at 6 months after treatment,and the difference was statistically significant(P<0.05).3 There was no significant difference between the observation group and the patients with positive bacterial vaginitis before and after treatment(P>0.05).The positive rate of bacterial vaginitis was significantly lower in the other groups before and after treatment(P<0.05).4 There was no significant difference in the negative rate of HPV in the observation group at 3 months,6 months,and 12 months after treatment(P<0.05).The HPV negative rate in the other groups after treatment was higher than that before treatment.Statistical differences(P<0.05).There were differences in the conversion rate of HPV at 3 months,6 months,and 12 months after treatment.Cold knife conization had higher negative rate of HPV than other groups.The difference was statistically significant(P< 0.05).5 The cold knife cervical conization had a higher negative rate of HPV in different types,and the difference was not statistically significant(P>0.05).The number of HPV16 subtypes in the anti-HPV biological dressing group was significantly higher than that of the other groups(P<0.05).The other 12 negative rates of HPV in the group were compared,and the other 12 negative rates of HPV in the nocadia rubra cell wall skeleton for external use group were compared with other groups.The difference was statistically significant(P<0.05).6 The anti-HPV bioprotein dressing had higher negative rate in HPV infection and low grade intraepithelial lesion than other groups,and the difference was statistically significant(P<0.05).7 vaginal pH,lactobacillus,bacterial vaginosis is related to HPV negative factors,the test differences were statistically significant(P <0.05).Conclusions 1 The application of anti-HPV drugs and surgical treatment can improve the vaginal microecology.2 Improvement of the vaginal microecology can increase the negative rate of HR-HPV.3 The application of vaginal anti-HPV drugs and surgical treatment can increase the negative rate of HPV.4 The anti-HPV protein dressing had a good negative effect on the HPV16 type,and the external Nocardia cell wall skeleton had better effect on other HPV type 12 negative transformation.The cold knife cervical conization was better for all types of HR-HPV negative transformation.
