Objectives To explore the effect of various systolic blood pressure trajectories on cerebral hemorrhage. Methods There were 57927 participants who underwent the first, the second and t he third physical examinations.A total of 55194 participants were enrolled in the s tudy.Proc Traj of SAS software was used to identify systolic blood pressure traject ories.we calculate the incidence rate of cerebral hemorrhage in different systolic bl ood pressure trajectory groups, and Log-Rank test was employed to compare the dif ferences in the cumulative incidence rate of cerebral hemorrhage among the groups.Cox proportional hazards models were used to analyze the effect of systolic blood pressure trajectory on cerebral hemorrhage. Results The mean age of participants was(49.36±11.90)years including 42536(77.1%)male.participants were classified into five groups: low-stable group,normal-st able group,moderate-high group,high-moderate group and high-stable group. The pa rticipants were followed up for an average of(5.99±0.41)years,and there were 164 cases of cerebral hemorrhage. The incidence rate of cerebral hemorrhage was 1.04,4.06,16.09,11.74 and 15.72/million years in low-stable group,normal-stable grou p,moderate-high group,high-moderate group and high-stable group,respectively(P<0.001).Cox proportional hazards models showed that after adjusting for confoundi ng factors, compared with the low-stable group, the risks of cerebral hemorrhage in normal-stable group,moderate-high group,high-moderate group and high-stable gro up were all increased, and the hazard ratios(95% confidence interval)were 3.69(1.69~8.05),15.08(6.48~35.11),2.01(4.50~32.09),17.22(5.80~51.14),respectively.Conclusions High systolic blood pressure trajectories are independent risk factors for cerebral hemorrhage.
