Objectives To observe the value of serum ferritin(SF)and fibrinogen(FIB)in patients with acute coronary syndrome(ACS)in Chinese.Methods Randomly selected patients(n=185),who had acute coronary syndrome in the first ward of Department of Cardiovascular Medicine of Tangshan Gongren Hospital Between 2015-09 and 2017-01.According to diagnostic classification,89 patients with non-ST-segment elevation acute coronary syndrome(NSTE-ACS)and 96 ST-segment elevation myocardial infarction(STEMI)patients.In the control group,60 cases in the negative control group were diagnosed as coronary artery disease in the same period,coronary artery spasm and myocardial bridge X syndrome were excluded by coronary angiography,and the stenosis < 50%.All the patients were local residents underwent emergency or selective coronary angiography and detected ferritin and fibrinogen.Coronary artery lesions were evaluated according to Gensini score.The general information and risk factors of the selected patients were inquired and recorded in detail.The electrocardiogram(ECG)of 12 or 18 lead,echocardiography and so on were improved.According to the patient's gender,race,age at admission,serum creatinine,and so on,The glomerular filtration rate(GFR)was calculated and obtained.According to the Grace risk score,the patients with ACS were ranked and divided into low risk group(? 108 scores,50 cases),moderate risk group(109-140 scores,80cases),high risk group(> 40 scores,55 cases).Statistical analysis using SPSS 19.0 data analysis.Objective to analyze the difference of serum ferritin and fibrinogen level between different clinical types,Grace risk score and Gensini score of ACS patients,and to analyze the relationship between serum ferritin and fibrinogen and risk factors of coronary heart disease.Results 1 Baseline data analysis showed that the proportion of history of hypertension and diabetes in ACS patients was higher than that in negative contrast group(pless than 0.05).The blood lipid parameters of total cholesterol,and low-density lipoprotein cholesterol ACS group were significantly higher than those of the control group(pless than 0.05).Correlation study showed,The correlation coefficient were r < 0.5,serum ferritin fibrinogen were not close hypertension,diabetes,low-density lipoprotein cholesterol levels.The level of FIB SF in ACS group were 3.90 ±0.67g/l,258.02 ±212.63ng/ml,and the serum FIB SF level in control group were 3.03 ±0.44g/l,118.56 ±49.60ng/ml,The level of serum FIB SF in ACS group was significantly higher than that in control group(P < 0.05,P < 0.05).2 The concentration of FIB SF and the diagnostic classification showed that STEMI > NSTE-ACS > control group(P < 0.05).3 The level of FIB SF in the high Gensini score group was significantly higher than that in the low Gensini score group,and that in the low Gensini score group was higher than that in the control group(P < 0.05).The risk stratification study of Grace score showed that FIB concentration was high risk group > middle risk group > low risk group > control group(P < 0.05).The concentration of SF in the high risk group was higher than that in the other groups(P < 0.05),and in the middle risk group,the low risk group was higher than that in the control group(P < 0.05).There was no significant difference between the middle risk group and the low risk group(P > 0.05).4 Correlation study showed that there was a positive correlation between FIB and Gensini score and Grace score in patients with ACS(r = 0.555,r = 0.327,P < 0.001).SF was positively correlated with Gensini score and Grace score(r = 0.520,r = 0.288,P < 0.001).5 The areas under the ROC curve for the diagnosis of ACS were 0.819 0.863,The results suggest that ferritin and fibrinogen are valuable in predicting the occurrence of acute coronary syndrome.Conclusions 1 The FIB SF concentration in patients with ACS was significantly higher than that in the control group.There was a positive correlation between FIB SF concentration and Gensini score,which was related to the severity of ACS,which was helpful for diagnosis and classification.2 SF FIB detection can provide useful information for early screening and risk stratification assessment in ACS patients.
