Objectives The objective is to Investigate the characteristics of blood sugar fluctuation and oxidative stress in patients with type 2 diabetes mellitus and obstructive sleep apnea hypopnea syndrome,meanwhile study the correlation between blood sugar fluctuation and oxidative stress,for providing a more comprehensive basis for the treatment of type 2diabetes with obstructive sleep apnea hypopnea syndrome.Methods 75 patients with type 2 diabetes mellitus combined with OSAHS were selected from diabetes with OSAHS group in Tangshan Fengnan District Hospital from April to December 2017.13 cases of health checkup in the same period were selected as health group.15 OSAHS patients without type 2 diabetes were included in OSAHS.31 patients with type 2 diabetes without OSAHS were diabetic patients.Four groups of subjects were tested for total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),fasting plasma glucose(FPG),and glycated hemoglobin(Hb A1c)Detection of superoxide dismutase(SOD),malondialdehyde(MDA),and 8-iso-prostaglandin F2?(8-iso-PGF2?)levels in four groups;the other patients in the diabetes with OSAHS group and the other three groups On the basis of dynamic glucose monitoring,mean amplitude of glycemic excursions(MAGE),Mean of daily differences(MODD),mean blood glucose(MBG),and standard deviation of blood glucose level(SDBG)were calculated.The FPG,Hb A1 c,MDA,8-iso-PGF2?,and SOD levels were compared between the diabetes with OSAHS group and the health group,the OSAHS group,and the diabetic group,respectively.The correlation of FPG,Hb A1 c,MAGE,and MODD with SOD,MDA,and 8-iso-PGF2 a was studied.SPSS19.0 statistical analysis software package was used for statistical analysis.The data of measurement data were expressed as(mean ± standard deviation),and comparisons among multiple groups were made using one-way ANOVA + multiple comparisons of HSD-q test.Count data use case number or rate,group comparison using ?2 test.In addition,Pearson correlation test was used to analyze the correlation between blood glucose fluctuations and oxidative stress indicators.Test level ?=0.05.Results 1 Diabetes with OSAHS group were compared with healthy group,OSAHS group and diabetes group,the age of the diabetes with OSAHS group(F=1.282,P=0.200),gender(?2=2.294,P=0.087),smoking history(?2=2.971,P= 0.085),years of education(?2=3.817,P=0.051),duration of diabetes(F=0.675,P=0.500),SBP(F=1.266,P=0.214),DBP(F=1.231,P=0.220),TG(F=1.110,P=0.341),TC(F=1.1.67,P=0.302),HDL-C(F=1.132,P=0.322),LDL-C(F=1.159,P=0.314)There was no statistical difference between the data,P>0.05.The BMI of diabetes mellitus combined with OSAHS group was higher than that of healthy group,OSAHS group,and diabetes group,and the difference was statistically significant(F=2.813,P=0.005).The FPG and Hb A1 C levels in diabetes mellitus group with OSAHS were higher than those in healthy group,OSAHS group,and diabetes group,the difference was statistically significant(F=3.098,P=0.002,F=3.442,P=0.001).2The levels of MAGE,MODD,MBG and SDBG in diabetes mellitus group with OSAHS were higher than those in healthy group,OSAHS group,and diabetes group.The data were statistically significant(F=10.121,P=0.000,F=9.442,P= 0.000,F=5.817,P=0.000,F=6.231,P=0.000).MAGE,MODD,MBG,and SDBG blood glucose fluctuations in the diabetes with OSAHS group and healthy group were compared with those in the OSAHS group.The data of the diabetes group were statistically significant,F=10.121,P=0.000,F=9.442,P=0.000,F=5.817,P=0.000,F=6.231,P=0.000.3 The content of SOD in diabetes with OSAHS group was 70.18 + 9.73U/ml,MDA content was 5.79 + 0.94mmol/L,8-iso-PGF2 a content was 1.38 + 0.89mg/mmol,SOD level was lower than that of the other3 groups,MDA and 8-iso-PGF2 a level were higher than that of the other 3 groups(F=3.457,P=0.001,P=0.001).4 Pearson correlation analysis was used to test blood glucose fluctuations MAGE,MODD,MBG,SDBG and SOD,MDA,8-iso-PGF2 a.The results showed that there was a positive correlation between the indexes of blood glucose fluctuation MAGE,MODD,MBG,SDBG and MDA and 8-iso-PGF2 a,and MODD,MBG,SDBG and SOD were negatively correlated.Conclusions 1 The average blood glucose fluctuations and average absolute blood glucose levels in patients with type 2 diabetes mellitus and obstructive sleep apnea-hypopnea syndrome are at a high level.2 The levels of malondialdehyde and 8-isoprostaglandin F2 a in patients with type 2 diabetes mellitus complicated with obstructive sleep apneahypopnea syndrome were higher,but the level of superoxide dismutase was relatively lower.3 There is a positive or negative correlation between glycemic fluctuation and oxidative stress in patients with type 2 diabetes mellitus and obstructive sleep apnea hypopnea syndrome.
