Objectives In order to investigate the distribution of polymorphism of G-2548A(rs7799039)in the promoter region of leptin gene,meanwhile,to explore the relationship between G-2548 A polymorphism and type 2 diabetic nephropathy,and provide theoretical basis for preventing and helping diabetic nephropathy.Methods 280 patients with type 2 diabetes admitted to the endocrinological area of North China University of Technology Affiliated Hospital from November 2016 to November 2017 were selected as the study subjects.According to whether they were associated with diabetic nephropathy or not,the patients were divided into diabetic nephropathy(DN)group of 115 and patients without diabetic nephropathy(NDN)of 165.Total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),and low-density lipoprotein cholesterol(LDL-C)were measured by professional technicians.Leptin(LEP)levels were measured by ELISA.The leptin gene G-2548 A polymorphism was detected by polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP)method.The database was entered using an Excel spreadsheet and spss17.0 was used.Measurement data was checked by using t test,and was expressed with mean ± standard deviation.The count data was tested with ?2 and represented by n(%).Whether or not DN was combined as a dependent variable,and the G-2548 A genotype in the promoter region of the leptin gene,body mass index,glycosylated hemoglobin,leptin,systolic blood pressure,diastolic blood pressure,and duration of diabetes were taken as independent variables,and multivariate logistic regression analysis was used to screen diabetes.Risk factors for kidney disease.P<0.05 indicates statistical significance.Results 1 There was no significant difference in age,triglyceride,cholesterol,lowdensity lipoprotein cholesterol,and high-density lipoprotein cholesterol between the DN group and the NDN group(P>0.05).2 Compared with BMI,Hb A1 c,leptin levels,systolic blood pressure,diastolic blood pressure and diabetes duration,the DN group was higher than the NDN group(P<0.05).3 The distribution frequencies of AA,AG and GG genotypes in NDN group were 43.6%,42.4%,and 13.9%,respectively.In the DN group,the frequencies of the three genotypes were 52.1%,36.4%,and 11.4%,respectively.The distribution frequency of AA genotype in the group was higher than that in the NDN group.There was a statistically significant difference in the distribution frequency of AA genotype between the two groups(?2 = 6.409,P = 0.041).4 The distribution frequencies of A and G alleles in NDN group were 64.8% and 35.2% respectively,in DN group were 78.3% and 21.7% respectively.The distribution frequency of A allele in DN group was higher than that in NDN group,and A allele was between the two groups.The frequency of gene distribution was statistically significant(?2 =11.691,P=0.001).5 Multivariate logistic regression was performed with or without diabetes nephropathy as the dependent variable and the genotype promoter G-2548 A genotype,body mass index,leptin,glycated hemoglobin,systolic blood pressure,diastolic blood pressure,and diabetes duration as independent variables.The analysis showed that the risk of developing diabetic nephropathy in patients carrying the AA genotype was 3.070 times(95% CI: 1.129 to 8.347),which was higher than that of the GG genotype.The difference was statistically significant(P=0.028),indicating the AA genotype.It is an independent risk factor for diabetic nephropathy.Conclusions 1 In the diabetic nephropathy group,there are different genotypes of the leptin gene promoter region G-2548 A,and the frequency of genotype distribution differs from that of the diabetic nephropathy group.2 The AA genotype of the 2548 locus of the leptin gene promoter may be a susceptible genotype of diabetic nephropathy.
