| Opinions on Deepening Healthcare System Reform,which was officially proclaimed in 2009,highlights the goal of universal coverage in terms of basic social medical insurance,which means insure all Chinese citizens.As the Healthcare Reform Plan is moving forward,three schemes,including basic social medical insurance scheme for urban employees,basic social medical insurance for urban residents program and the New Rural Cooperative Medical Insurance for Rural Residents program,have been established and are backbone for the Basic Social Medical Insurance(BSMI)System in China.However,the system appears fragmented due to the following factors which include the economic development varied across regions within China,imbalance in medical resource distribution,overall planning at low levels and the autonomy to make adjustments by local governments etc.As the population becomes more mobile between regions while the scale of mobile population is expanding,the number of the insured who receive medical treatment in different places is increasing rapidly,and causes the soaring medical costs for medical treatment in different places which will affect the normal operation and development of the medical insurance system and result in uncontrollable risks in medical cost.The government exercises the state power for people,so livelihood issues related to people are never bit of petty things,the issues about the insured receiving medical treatment in different places and soaring medical costs now become urgent problems to be solved by provincial governments.As a pilot province participates in the healthcare reform,Jilin Province actively carries out all the state policies,and aims at providing a more accessible,efficient and accurate management on medical insurance system.The introduction of this paper will elaborate the background and significance of the subject,illustrate the research contents and methods,as well as the innovation and flaws of this paper and then sort out some domestic and international references.Secondly,the paper will define some basic concepts,and provide theoretical basis for controlling the medical costs.Thirdly,the paper will enumerate policies on remote medical treatment drafted by Jilin provincial government and review the implementations;describe the growth of medical costs for remote medical treatment in Jilin Province through analyzing relevant data from January 2010 to September 2015.Fourthly,based on viewpoints of reasonable and unreasonable factors that affect the growth of medical costs for remote medical treatment,the paper figures out that reasonable factors which consist of medical insurance expansion,implementation of policies that provide convenience to remote medical treatment,large aging population of the insured who receive medical treatment in different places,strong awareness of health,the application of new medical technologies and equipment;while the unreasonable factors include difficulties in collaboration management of remote medical treatment,moral hazard triggered by suppliers and demanders of remote medical services,difficulties in monitoring personal information of remote medical treatment,and imbalance in medical resource distribution.Finally,by applying demand and supply theory on basic social medical insurance,supplier-induced demand(SID)theory and moral hazard theory,the paper will provide four accessible suggestions on how to control unreasonable expenditures of basic social medical insurance under the situation of remote medical treatment,which mean to improve collaboration management of remote medical treatment,promote controlling schemes on moral hazard,enhance monitoring system of remote medical treatment information,cement multilevel medical and health system,and move the hierarchical medical system forward. |
