Objective: The value of pleural effusion in the treatment of severe acute pancreatitis by combined CT imaging technique was explored to improve the therapeutic effect and prognosis of patients with severe acute pancreatitis,and reduce the incidence of severe complications and mortality of patients with severe pancreatitis.Methods: We selected 136 patients with severe acute pancreatitis complicated with pleural effusion from January 2016 to October 2017 in Changde First People Hospital.According to the amount of pleural effusion,patients with severe acute pancreatitis were divided into three groups: a small amount of pleural effusion groups,a medium amount of pleural effusion groups,and a large amount of pleural effusion groups were scanned using a GE VCT 16-slice 32-slice spiral CT machine to collect sex,age,diabetes,hypertension,coronary heart disease,hyperlipidemia,biliary stones,BMI,48-hour Ranson score,and APACHE II score,hospitalization days,pancreatic pseudocyst,necrotizing pancreatitis,pancreatic abscess,acute respiratory distress syndrome,shock,incidence of acute renal insufficiency,and mortality..The data obtained in this study were recorded and collated by Excel 2003 software,and statistical analysis was performed using Empower software and R software.Results: The baseline data of patients with severe acute pancreatitis and pleural effusion were collected and compared.The results showed that the three groups of patients with severe acute pancreatitis with pleural effusion had no diabetes,hypertension,coronary heart disease,high blood lipids,and biliary stones.There was a statistical difference(P>0.05).There was no significant difference in gender,BMI between the small,middle and large groups of pleural effusions(P>0.05),and there were significant differences in length of hospital stay,Ranson score,APACHE II score and age(P <0.05),That is,with the age of patients with severe acute pancreatitis,the greater the amount of pleural effusion and the longer the length of hospital stay,the higher the Ranson score and APACHE II score.Pleural effusions were also found in a small number of groups,a medium-sized group,and a large group of three groups,including pancreatic pseudocyst,necrotizing pancreatitis,pancreatic abscess,acute respiratory distress syndrome,shock,acute renal insufficiency,and death.Sex differences(P<0.05),that is,the number of cases with pancreatic pseudocyst,necrotizing pancreatitis,pancreatic abscess,acute respiratory distress syndrome,shock,acute renal insufficiency,and death as the amount of pleural effusion increased many.The complications of patients and the influencing factors of prognosis were analyzed.Age,diabetes,hypertension,coronary heart disease,hyperlipidemia,biliary stones,BMI,Ranson score,APACHE II score and quantification of pleural effusion were used as independent variables.Complications and prognosis were used as dependent variables.Multiple stepwise Logistic regression analysis showed that the age,pleural effusion volume,Ranson score and APACHE II score were independent risk factors for complications and prognosis in patients with severe acute pancreatitis.Conclusions: 1.Quantification of pleural effusions by CT examination is helpful for assessing the severity of patients with severe acute pancreatitis1;2.The amount of pleural effusion is related to the occurrence of complications and prognosis in patients with severe acute pancreatitis,and with the larger the amount of pleural effusion,the more serious complications of patients,the prognosis is worse,the higher the mortality rate.
