Ulcerative colitis(UC)is a kind of rectum and colitis disease with the main clinical manifestations of diarrhea,mucous purulent blood stool,abdominal pain and acute and severe.The pathogenesis of the disease may be caused by genetic,immune,infection,mental and other factors,so it is often recurrent and difficult to cure,and has a certain relationship with the incidence of colorectal cancer.Therefore,the active treatment of this disease has a very important clinical significance.In this study,the UC rat model was used as the study object.By orthogonal experimental design method,the dose-effect analysis was performed on the composition of Kuijieshuang enema prescription which enema treatment for UC,and the best combination of kujieshuang enema prescription was selected.Further,through clinical observation to determine its efficacy.The Kuijieshuang enema prescription drug components: Sophora flavescens,Burnet,Phellodendron chinense and Bletilla striata were designed as three levels.The L9(34)orthogonal table was selected for orthogonal test design,and the No.1 to No.9 experimental drugs were obtained.65 healthy 8-week-old male Wistar rats were selected for experience.Except for the normal group rats,all the other groups were treated with dextran sulfate sodium(DSS)by intragastric administration to prepare the UC rat model.After the model was established,the prescription was given orally by referring to the orthogonal table.The course of treatment was 10 days.Observe the pathological score of colon tissue in UC rats,and analyze the dose-effect relationship of the test data,and screen out the best enema group for treating UC,namely New Kuijieshuang enema prescription: Sophora flavescens 12 g,Burnet 20 g,Phellodendron chinense 12 g,Bletilla striata 10 g.Then,50 wistar rats were selected and divided into model group,Kuijieshuang group,New Kuijieshuang group and mesalazine group,except for 10 normal rats.A total of 4 groups,10 in each group,Treatment with Kuijieshuang enema prescription,New Kuijieshuang enema prescription and mesalazine enema respectively.The treatment course was 10 days.The general condition,DAI scores,serum CRP,IL-17 levels and histopathology scores of the rats were observed,then analysis and statistics of experimental data.The results showed that compared with the original formula,New Kuijieshuang enema prescription can significantly reduce the expression of CRP and IL-17 in UC rats,and improve the lesions of colon mucosa in UC rats.The curative effect is significant(P<0.05).After that,48 outpatients and hospitalized UC patients in our hospital were randomly divided into two groups,the treatment group and the control group,24 in each group.The treatment group was given New Kuijieshuang enema prescription combined with oral treatment by traditional Chinese medicine syndrome differentiation,the control group was given mesalazine enema combined with mesalazine for oral treatment.The course of treatment was 12 weeks.The diarrhea,pus,blood diarrhea,abdominal pain,and colonoscopic changes were observed before and after treatment.The results showed that the diarrhea,blood diarrhea,and abdominal pain scores of the two groups were significantly lower than those before treatment(P<0.05).The Chinese medicine group was significantly better than the Western medicine group in improving clinical symptoms such as diarrhea,pus and blood stagnation,and the difference is significant(P<0.05);In improving the symptoms of abdominal pain,there was no significant difference between the Chinese medicine group and the western medicine group(P>0.05).The effective rates of the Chinese medicine group and the western medicine group were 78.26% and 76.19%,respectively,indicating that both the Chinese medicine and Western medicine can significantly improve the condition of the colon mucosa,but there is no significant difference between the two groups(P>0.05).To sum up,through animal experiments and clinical case observations,we can conclude that patients with mild and moderate ulcerative colitis involving the rectum and rectum sigmoid colon,Xinkuijie Shuang(Sophora flavescens 12 g,Burnet 20 g,Phellodendron chinense 12 g,Bletilla striata 10g)had definite curative effect.
