Hypertensive disorder complicating pregnancy is a kind of disease of increaseing blood pressure in pregnancy at the same time,and these diseases cause the whole body viscus blood flow decreases,harm to female health,and even cause death.Use the TDI-Tei index,shortening fraction and inferior vena cava concerned index to evaluate the right ventricular function of the fetuses with hypertensive disorder complicating pregnancy.59 fetuses with HDCP(disease group)and 120 fetuses without HDCP(control group)were selected from 28 th week to 39 th week with six day gestation.And all the fetuses was divided into three groups,28 th week to 31 th week with six day gestation,32 th week to 35 th week with six day gestation,36 th week to 39 th week with six day gestation.Firstly,use TDI to measure the isovolumic contraction time(ICT),isovolumic relaxation time(IRT)and ejection time(ET)of right ventricle in each fetus,use the formula(ICT+IRT)/ET calculate right ventricular Tei index,and measure three consecutive cardiac cycles,take the average.Then,measure fetal right ventricular end-diastolic diameter(EDD),fetal right ventricular end systolic diameter(ESD),use the formula: SF =(EDD-ESD)/EDD calculate right ventricular SF,and measure three consecutive cardiac cycles,take the average.Last,measure inferior vena cava concerned index,including: ventricular systolic peak velocity,Vmax(S),ventricular diastolic peak velocity,Vmax(D),atrial systolic peak velocity,Vmax(A),and measure three consecutive cardiac cycles,take the average.Totlely,the fetal right ventricle function was evaluated by the above parameters.The results showed as follows:1 Maternal age,gestational age and fetal heart rate had no difference between the two groups(P>0.05).2 There was no correlation between fetal right ventricular Tei index and heart rate in the control group(r=0.009,P>0.05).3 the success rate of the all fetal TDI-Tei index and inferior vena cava concerned index is about 97.2%.4 The inferior vena cava concerned index of the fetuses with HDCP was positively correlation with right ventricular Tei index[Vmax(S)/Vmax(D):r=0.564,P<0.05;Vmax(A)/Vmax(S):r=0.621,P<0.05].5 Compared to the control group,the 28 th week to 31 th week with six day gestation disease group fetal TDI-Tei index and inferior vena cava concerned index are higher(P<0.05),shortening fraction have no significant change with control group(P>0.05).6 Compared to the control group,the 32 th week to 35 th week with six day gestation disease group fetal TDI-Tei index and inferior vena cava concerned index are higher(P<0.05).shortening fraction have no significant change with control group(P>0.05).7 Compared to the control group,the 36 th week to 39 th week with six day gestation disease group fetal the TDI-Tei index and inferior vena cava concerned index are higher(P<0.05),shortening fraction have no significant change with control group(P>0.05).Tei index is a doppler index of evaluating right ventricular diastolic function and systolic function,shortening fraction(SF)is a traditional index of evaluating ventricular systolic function,inferior vena cava connect directly to the right atrium,the blood flow is influenced by right heart function change,so can directly or indirectly reflect the change of fetal heart diastolic function of right.The TDI-Tei index?shortening fraction and inferior vena cava concerned index of fetuses with HDCP can be taken for clinical evaluation about the fetal ventricular function.
