Objective:The aim of this study was to understand the status of Post-stroke fatigue(PSF)and self-efficacy in patients with ischemic stroke,to analyze the influencing factors of PSF and self-efficacy in patients with ischemic stroke,to explore the correlation between PSF and self-efficacy in patients with ischemic stroke, and to provide reference for patients with ischemic stroke to increase self-efficacy and relieve post-stroke fatigue. Methods:This study was a correctional study during March to December 2017,228 patients with first-ever ischemic stroke who were hospitalized in the Department of Neurology of a top-three hospital in Changchun,Jilin Province were selected to participate in this study. We used the general information questionnaires,Fatigue severity scales(FSS),Modified Rankin Scale(MRS score),Stroke Self-Efficacy questionnaire(SSEQ),Self-Rating Depression Scale(SDS),and Perceived Social Support Scale(PSSS)to conduct Follow-up surveys at 3 months after stroke onset.Epidata3.1 was used to enter data, and SPSS 23.0 statistical analysis software was used for statistical description,t-test,one-way analysis of variance,Pearson correlation and multiple stepwise regression analysis.Results:1.Post-stroke fatigue in patients with ischemic stroke: The overall average of FSS was(3.83±1.27)scores at 3 months after stroke and there were 108 patients with the average of FSS ?4 scores,47.4% of stroke patients reported clinically significant levels of fatigue. The univariate analysis showed that PSF between degree of education, pathological changes, and MRS score were significantly differences in patients with ischemic stroke(P?0.05).There was a significant positive correlation between depression and PSF.The correlation coefficient was 0.543,P?0.01.The total score of social support, family support, support outside the family with PSF were significantly negatively related. The correlation coefficients were-0.508,-0.454,and-0.363 respectively,P?0.01.2.Self-efficacy after ischemic stroke: The overall average of self-efficacy was(63.39±18.89)scores at 3 months after stroke and the overall level was dominated by middle and lower levels, the aspect of daily living was(39.00±11.61)scores and the aspect of self-management was(24.40±7.27)scores;The univariate analysis showed that the level of self-efficacy of patients with ischemic stroke was statistically significant in terms of education level, monthly household income per capita, living conditions, and MRS scores,diabetes(P?0.05).There was a significant negative correlation between depression and stroke self-efficacy.The correlation coefficient was-0.264,P?0.01.Social support score, family support, support outside the family and stroke self-efficacy were significantly positively correlated. The correlation coefficients were 0.498,0.489 and 0.391 respectively,P?0.01.The results of multivariate analysis showed that education level, monthly household income per capita,MRS score and social support status were the main influencing factors of the self-efficacy of patients with ischemic stroke, which explained 58.7% of the self-efficacy variables.3.Correlation between self-efficacy and PSF in patients with ischemic stroke: Pearson correlation analysis showed that there was a significant negative correlation between self-efficacy,daily activities and self-management with post-stroke fatigue. The correlation coefficients were-0.654,-0.587 and-0.612 respectively,P?0.01.In the multivariate linear stepwise regression model, stroke self-efficacy was independently associated with post-stroke fatigue(?=-0.371,P?0.001)after controlling the patient's demographic and disease-related variables,as well as basal ganglia lesions,MRS score, the level of depression, social support explained 71.7% of the PSF variables.Conclusion:1.Post-stroke fatigue is more common in patients with ischemic stroke 3 months after stroke onset, and the main factors that could affect PSF were basal ganglia lesions, physical dysfunction, depression and social support.2.The self-efficacy of patients with ischemic stroke was generally in a moderate and lower level 3 months after stroke onset, and the main factors that could affect self-efficacy were education level, monthly household income per capita, physical dysfunction and social support.3.There was a significant negative correlation between self-efficacy and PSF in patients with ischemic stroke after controlling for lesions,MRS score, depression and social support. The lower the stroke self-efficacy,the higher the post-stroke fatigue.
