OBJECTIVE:Comparison of curative effects between treatment of adrenal corticosteroids and glucocorticoids on West syndrome in controlling seizures and recurrences and EEG changes.METHODS:Computer-based searches of Pubmed,Cochrane,Embase,CNKI,Wanfang database,China Biomedicine database,and manual compilation of conference dissertations and monographs collected RCTs from January to December 2017 for all relevant studies literature.The collected documents were read in detail,excluding duplication,non-compliance with the purpose of this study and low-quality literature,screening the literature using the Cochrane Collaboration systematic review and Meta-analysis of the incorporated literature using Review Manager 5.3 software.RESULTS:Meta analysis of the included literature was performed using Review Manager 5.3 software.In the comparison of adrenocorticotropic hormone and glucocorticoid control in infantile spasm,a total of 16 groups of data,to remove the more heterogeneous literature,a total of 13 groups of data were included,the study included 781 cases of research subjects,including experimental group(application Adrenocorticotropic hormone was 380 cases and control group(corticosteroids)401 cases.Meta analysis using Review Manager 5.3 software.The results showed that:(OR=0.78,95% confidence interval [0.57,1.06] P=0.11),studies have shown that in controlling infantile spasms,preference is given to glucocorticoids superior to adrenocorticotropic hormone,but the difference between the two groups of data is Statistical significance;in the comparison of corticosteroids and glucocorticoids in the control of high amplitude rhythm of EEG in children with West syndrome,a total of 8 sets of data,the study included 487 subjects,including experimental group(adrenal Corticosteroids(266 cases)and control group(corticosteroids)(221 cases).Meta analysis using Review Manager 5.3 software.The results showed that:(OR=1.30,95% confidence interval [0.88,1.92] P=0.19),studies have shown that in favor of controlling the high amplitude rhythm of EEG in children with West syndrome,it tends to adrenocorticotropic hormone better than sugar Corticosteroids,but the difference between the two groups of data was not statistically significant;in the comparison of corticosteroids and glucocorticoids control the recurrence of clinical spastic episodes in children with West syndrome,the study included a total of five groups of data,in the elimination of heterogeneity After the large literature,the study included a total of 4 groups of data,including 124 subjects,of which 44 were in the experimental group(adrenocorticotropic hormone)and 80 in the control group(glucocorticoids).Meta analysis using Review Manager 5.3 software.The results showed:(OR=2.40,95% confidence interval [1.03,5.59] P=0.04).Studies have shown that adrenocorticotropic hormone is superior to glucocorticoids in the control of post-pallial recurrence in children with West syndrome.The difference is statistically significant,but the included literature and the total sample size are small and still require a large number of clinical trials.Supplementary test conclusions.CONCLUSIONS:Adrenocortical corticosteroids and glucocorticoids had no statistically significant differences in the control of spasmodic episodes in children with West Syndrome and in the improvement of EEG peak rhythm disturbance in children.Adrenocorticotropic hormone is superior to glucocorticoids in the control of recurrence after hernia.
