Background and ObjectiveThe study had shown that Epicardial Adipose Tissue(EAT)is associated with the occurrence and maintenance of atrial fibrillation.However,there are few studies on the relationship between EAT and ventricular arrhythmias.The purpose of this study was to investigate the relationship between volume of periventricular EAT and ventricular premature beat by measuring the volume of epicardial adipose tissue in different parts and repolarization related indexes(QT,Tp-e interval and Tp-e/QT ratio).MethodsA total of 59 patients with frequent ventricular premature beats were achieved 256-row spiral computed tomography(CT)cardiac enhancment scanning as experimental group in the cardiovascular center of No.1 Hospital of Jilin University from June 2017 to February 2018.In the same period of time,55 patients without coronary heart disease by undergoing cardiac 256-row spiral coronary artery computed tomography angiography(CCTA)who had no ventricular premature beats at the outpatient department of the First Hospital of Jilin University in the control group.1)All patients are recorded their age,body mass index(BMI),diabetes(yes/no)hypertension(yes/no)history,blood routine and blood biochemical indicators.2)The original data of cardiac enhanced CT or CCTA were transmitted to the PHILIPS workstation for three-dimensiona post-processing,and Volume software was used to analyze and calculate the volume of periventricular EAT in different parts of the ventricular epicardium.3)The QT,Tp-e interval and Tp-e/QT ratio were measured respectively.Results1.Compared with the control group,the experimental group had no statistical significance in age,gender,body mass index,diabetes,hypertension,blood routine,and blood biochemical indicators,P>0.05;2.Compared with the control group,the median total volume of the EAT in the experimental group was [69.3(55.0,84.7)cm3 vs 53.8(48.7,60.0)cm3],the median volume of the left ventricular EAT [39.1(33.7,56.9)cm3 ratio 32.7(24.0,49.5)cm3],and the median volume of the right ventricular EAT [21.1(19.2,31.2)cm3] were increased,the difference was statistically significant,P <0.01.Compared with the control group,the median Tp-e interval in the experimental group [151(140,160)ms vs 112(100,120)],the median QT interval [454(430,470)ms vs 391(386,420)ms],Tp-e/QT median [0.36(0.34,0.40)vs 0.31(0.27,0.35)] were increased,the difference was statistically significant,P <0.01.3.The number of ventricular premature beats in experimental group was positively correlated with the total,right and left volume of periventricular EAT.The correlation coefficients were(r=0.608,0.737,0.535).P <0.001.4.Quantify the volume of PAT into four groups,less than 25%,25%-50%,50%-75%.More than 75% are defined as 1,2,3,and 4.The Tp-e interval from first groups to fourth groups(108.53 + 31.89 ms vs 133.70 + 22.29 ms vs 135.92 + 18.01 ms vs 145.65 + 20.42ms),QT interval from first to fourth groups(396.7 + 31.19 ms vs 412.03 + 42.15 ms vs 429.93 + 30.87 ms vs 442.66 + 34.47ms)and the ratio of Tp-e/QT(0.29±0.08 vs 0.35±0.04 vs 0.37±0.03 vs 0.39±0.04)were increased,the difference was statistically significant,P < 0.001.ConclusionThe volume of periventricular EAT is closely related to the occurrence of ventricular premature beats.The mechanism may be related to the increase in the volume of pericardial EAT that leads to prolonged ventricular repolarization.
