| Part1 Comparative study of epicardial adipose tissue volume in patients with atrial fibrillation and normal sinus rhythm by 256-slice CTObjective: To explore the correlation between the volume of epicardial adipose tissue volume(EATV)and atrial fibrillation(AF)and its clinical value by 256 slice spiral CT.Method:A total of 66 AF patients were enrolled and divided into 2 groups: Paroxysmal AF group(n=42)and Persistent AF group(n=24),there were 38 males and 28 females,with an average age of 60.11±9.83 years;In addition,70 cases with sinus rhythm were also enrolled as control group,there were 40 males and 30 females,with an average age of 59.99±10.59 years.All patients were performed by 256 slice spiral CT,EATV and pericardial adipose tissue volume(PATV)were measured using cardiac function software in workstation,and basic information of all cases were recorded.EATV,age and sex were compared between the AF group and the control group.EATV,PATV and other clinical data were further analyzed between the paroxysmal AF group and persistent AF group.Results:1.AF group had higher EATV compared with control group(P<0.001).2.EATV and PATV in persistent AF group were slightly higher than those in paroxysmal AF group,but there was no significant difference between the two groups(P>0.05).3.There was a significant positive correlation between EATV and PATV in AF group(r=0.953,P<0.001).4.Logistic regression analysis indicated that EATV(OR=1.143,95%CI: 1.027-1.158,P=0.000)was independently associated with AF.5.The AUC of EATV to predict AF was 0.822(P<0.001),When the cutoff value of EATV was 97.25 ml,the sensitivity and specificity of the diagnosis of AF were 93.8% and 61.4% respectively.Conclusion:EATV can be objectively and quantitatively measured by 256 slice spiral CT.The increase of EATV was closely related to the occurrence of AF.However,there was no significant difference in EATV between different types of AF group.EATV is significantly related to the occurrence of atrial fibrillation,and is an important index to predict the occurrence of atrial fibrillation.Part2 Study on the correlation between atrial fibrillation and obesity、BMI and epicardial fat by 256-slice CTObjective: To explore the correlation between epicardial fat volume and obesity and atrial fibrillation.Method:A total of 74 patients with atrial fibrillation from 2016-09 to 2018-January in our hospital were collected and divided into 2 groups: Paroxysmal AF group(n=47)and Persistent AF group(n=27),there were 43 males and 31 females,with an average age of 59.86±9.88 years;In addition,75 cases with sinus rhythm were also enrolled as control group,there were 44 males and 31 females,with an average age of 56.12±10.75 years.All patients were performed by 256 slice spiral CT,The epicardial fat volume of all patients was measured by post-processing software,and their weight,height and BMI were recorded.The correlation between the epicardial fat volume and the risk factors such as BMI and atrial fibrillation was evaluated by multivariate regression analysis.Results:1.The EATV of AF group(138.94 ±37.93 ml)and BMI(26.08 ±2.91)were significantly higher than those of control group(P<0.05).2.There was no significant difference in EATV and BMI between paroxysmal atrial fibrillation group and persistent atrial fibrillation group(p > 0.05).3.Logistic multivariate regression analysis showed that epicardial fat volume was associated with atrial fibrillation(OR=11.579,95%CI:6.595~33.315,P=0.000).4.According to Pearson correlation analysis,the EATV was positively correlated with BMI(r=0.554,P<0.05)and body weight(r=0.514,P<0.01).Conclusion: Increasing of EATV and obesity are associated with atrial fibrillation,EATV was more closely related to atrial fibrillation,was a risk factor associated with AF independent of age,sex and BMI.The volume of epicardial fat varies with weight loss,which is the main factor associated with obesity and atrial fibrillation. |
