Objective: through the introduction and establishment of a hospital community integrated management mode that accords with Xinjiang's regional characteristics and national characteristics,we should carry out diabetes prevention and control inminority areas under the guidance of integrated medicine and precision medicine.Under the intervention of model health management,the effect of management is analyzed and evaluated.To establish and implement the theoretical basis for the prevention and control of Type 2 diabetes in Xinjiang.Methods: the research carried out in the pilot pilot community hospital-the hospital community integrated management based on T2 DM,the management status using the descriptive analysis,archiving,analysis by F test with six month rate of three months,2 test was applied to the model before and after the implementation of patients with diabetes knowledge awareness rate comparison.Effect analysis of patients and high-risk groups: correlation analysis was conducted by Spearman correlation analysis,multivariate analysis and Logistic regression analysis were used to describe the results.Results:(1)management status: a total of 6313 patients were included in the evaluation,including 3123 men and 3190 women.3782 of the study population had complications,of which 2232 of the highest patients were hypertension.(2)according to the standard of comprehensive compliance management evaluation indicators qualified status,FBG standard 2931 people accounted for 37.9%,the blood rate: TG standard 797 people accounted for 12.6%,TC standard 658 people accounted for 10.4%,comprehensive compliance is not ideal;phase comparison: blood glucose(P<0.01),blood pressure(SBP.P<0.05;SDP,P>0.05),blood lipids(TG,HDL-C,P<0.05;TC,LDL-C,P>0.05),obvious hypoglycemic effect of blood glucose control,P<0.05,there was significant difference between each stage management effect.(3)T2DM knowledge awareness rate: ten problems.After health knowledge lecture hall and community intervention,the difference is(P<0.01).1,can diabetes cure 2 fasting blood glucose normal value 3,diabetes diagnosis pointer 4 blood related symptoms caused by diabetes.(4)analysis of the effect of management model: multiple factor Logistic regression analysis of FBG influencing factors.The degree of education below high school is an independent risk factor for FBG.The risk factors analysis of follow-up results showed that exercise was more than 4 times a week,more than 30 minutes per time was a protective factor for the satisfactory follow-up results.The main food intake was more than 200 g.(5)analysis of high risk factors of high risk population: SBP,DBP and waist circumference were risk factors of whether the heart function was qualified(OR>1).Conclusion: the integrated management of hospital and community is effective for the management of T2 DM,but the comprehensive control is short.Educational background is below the high school level,is the influence factor of fasting blood sugar not reaching the standard.Centripetal obesity is an important determinant of central blood pressure and arterial hardness.
