Objective:By measuring the expression of extracellular matrix metalloproteinase inducer(EMMPRIN)in peripheral blood of patients with coronary heart disease,and the expression levels of MMP14(matrix metalloproteinase 14)and MMP15(matrix metalloproteinase 15)in serum,To clarify the relationship between EMMPRIN and MMP14,MMP15.Combined with coronary angiography and 64-slice spiral CT coronary imaging,To clarify its significance in different plaque types,different coronary lesions,and different coronary lesions and provide theoretical basis for the pathogenesis and clinical treatment of coronary heart diseaseMethods:collecing a total of 116 patients with Coronary Heart Disease(CHD)diagnosed in the the General Hospital of Kunming General Hospital of PLA from May 2017 to October 2017,Another 38 cases of coronary angiography was normal those for the control group.According to the clinical diagnosis,CHD group was divided into stable angina pectoris(SAP)group 42 cases,unstable angina pectoris(UAP)group 36 cases,and acute myocardial infarction group(AMI)38 cases.According to the results of CT coronary examination,the CHD group was divided into 50 cases of soft plaque group,45 cases of fibrous plaque group and 21 cases of calcified plaque group-According to the lesion involving the blood vessel range.the CHD group was divided into single-vessel disease group 44 cases,double-vessel disease group 44 cases,three-vessel disease group 28 cases.Firstly,the platelet and monocytes were isolated by density gradient centrifugation,then the expression levels of EMMPRIN(MFI)on the cell surface of the two cells were detected by flow cytometry.Finally the expression level.of MMP14 and MMP15 were detected by enzyme-linked immunosorbent assay(ELISA).Results:1?Comparison of EMMPRIN expression on platelets and monocytes in SAP group,UAP group,AMI group and control group:The expression levels of EMMPRIN on platelet surface in SAP group,UAP group,AMI group and control group were significantly different(F=23.36,P<0.01).The comparison between groups showed that AMI group was higher than UAP group and SAP group.UAP group Compared with the SAP group,the differences were statistically significant(P<0.05);and the three groups were higher than the control group,and the differences were statistically significant(P<0.05);the EMMPRIN expression levels on the surface of monocytes were different among the groups.There was a statistically significant difference(F=14.913,P<0.01).The differences between groups were not statistically significant between the AMI group and the UAP group(P=0.27>0.05).The AMI group was higher than the SAP group and the difference was statistically significant.Significance(P<0.05);UAP group was higher than SAP group,the difference was statistically significant(P<0.05);and the three groups were higher than the control group,the difference was statistically significant(P<0.05).2.Comparison of serum MMP14,MMP15 expression levels in SAP group,UAP group,AMI group and control group:There were significant differences in MMP14 expression between SAP group,UAP group,AMI group and control group(F=4.130,P=0.009<0.05).There was no significant difference between AMI group and UAP group(P=0.504>0.05),AMI group was higher than SAP group,the difference was statistically significant(P<0.05);UAP group was higher than SAP group,the difference was statistically significant(P<0.05);The AMI and UAP groups were higher than the control group,and the differences were statistically significant(P<0.05).The difference between the SAP group and the control group was not statistically significant(P=0.953>0.05).The difference of MMP15 expression between SAP group,UAP group,AMI group and control group was statistically significant(F=3.198,P=0.028<0.05).The comparison between groups showed that AMI group was higher than UAP group and SAP group.The UAP group was higher than the SAP group,and the difference was statistically significant(P<0.05).The three groups were all higher than the control group,and the differences were statistically significant(P<0.05).3?Comparison of EMMPRIN Expression Levels on Different Plaque Types of Platelets and Monocytes:The expression levels of EMMPRIN on the surface of platelet and monocyte of different plaque types were statistically significant(F=24.536,P<0.01;F=25.521,P<0.01).Between the two groups,and the soft plaque group was more fibrous plaques.In the group of calcified plaques,the differences were statistically significant(P<0.05).The fibrous plaque group was higher than the calcified plaque group(P<0.05),and the three groups were all high.In the control group,the differences were statistically significant(P<0.05).4?Comparison of levels of MMP14 and MMP15 in different plaque types:There was a significant difference in MMP14 expression levels between different plaque types(F=34.684,P<0.01).Between the two groups,the soft plaque group was higher than the fibrous plaque group and the calcified plaque group,and the differences were statistically significance(P<0.05);There was no statistical difference between the fibroplasia group and the calcified plaque group(P=0.986>0.05),all three groups were higher than the control group,and the differences were statistically significant(P<0.05);There was a statistically significant difference in the expression level of MMP15 in the plaque type(F= 17.560,P<0.01).Compared between the two groups,the soft plaque group was higher than the fibrous plaque group and the calcified plaque group,and the fibrous plaque group was more In the calcified plaque group,the differences were statistically significant(P<0.05),and all three groups were higher than the control group,and the differences were statistically significant(P<0.05).5?Comparison of EMMPRIN Expression Levels on Platelet and Monocytes at Different Lesion Ranges:The expression of EMMPRIN on the surface of platelet and monocyte in different lesion range was statistically significant(F=27.250,P<0.01;F=32.524,P<0.01).Between the two groups,the three-vessel disease group was higher than the double-vessel disease group and single-vessel disease group,and the double-vessel disease group was higher than the single-vessel disease group,and the three groups were higher than the control group,the differences were statistically significant(P<0.05).6?Comparison of levels of MMP14 and MMP15 in different lesions:The difference of MMP14 levels between different coronary lesions was statistically significant(F=21.086,P<0.01).Between the two groups,the three-vessel disease group was higher than the double-vessel disease group and single-vessel disease group,and the double-vessel disease group was higher than the single-vessel disease group(P<0.05);the three-vessel disease was statistically significant(P<0.05);The double-vessel lesions were higher than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference between the single-vessel lesions and the control group(P>0.05).The difference of serum MMP15 expression levels between different coronary lesions was statistically significant(F=16.346,P<0.01).Between the two groups,the three-vessel disease group was higher than the double-vessel disease group and single-vessel disease group,and the double-vessel disease group was higher than the single-vessel disease group,and the three groups were higher than the control group,the differences were statistically significant(P<0.05);7?Correlation Analysis of EMMPRIN with MMP14,MMP15:A simple linear correlation analysis between serum MMP14,MMP15 and platelet surface EMMPRIN was performed in patients with coronary heart disease(r=0.782,P<0.00;r=0.876,P<0.00),and there was a positive correlation between them.A simple linear correlation analysis between serum MMP14,MMP15 and EMMPRIN on monocyte surface was performed in patients with coronary heart disease(t=0.745,P<0.00;r=0.896,P<0.00).There was a positive correlation between them.Conclusion(s):1?The expression of EMMPRIN,MMP14 and MMP15 is significantly increased in patients with ACS,indicating that they may play an important role in the development of ACS.2?Coronary CTA results suggest that in patients with ACS,the type of coronary plaque is mainly soft plaque,the expression of EMMPRIN,MMP14,and MMP15 is increased,indicating that the three are closely related to plaque instability.3?EMMPRIN was positively correlated with MMP14 and MMP15,indicating that EMMPRIN-MMP14 and EMMPRIN-MMP15 may regulate and interact with each other during the development of ACS.
