| Objective:Methadone Maintenance Treatment is the most effective method for heroin addicts so far.The Take-home Methadone Service is a novel service that gives patients a dose of methadone at home or where it will not be observed without requiring a return to the clinic.TMS]is based on the negative rate of urine selection,reflects the external objects are good compliance of the population.Gender analysis is widely used in sexual and reproductive health,such as health equity research,reproductive health research,and AIDS research.No research has been conducted on TMS from a gender perspective.The purpose of this research was to study methadone cognition,methadone treatment experience,drug compliance,urine positive rate,family function,social support and other demographic and sociological characteristics of patients with methadone extracorporeal by using the framework of gender analysis.To investigate the status of methadone maintenance compliance and its influencing factors.From the point of view of gender,does TMS meet the needs of women in the context of gender sensitivity.Whether to promote women's easier use of methadone services,to promote women's integration into the family and society,and whether women's compliance is better in methadone extracurricular services.To provide ascientific basis for the development of agender sensitive TMS.Methods:In this study,qualitative studies were used,supplemented by a mixture of epidemiological descriptive studies.The quality analysis and quantitative analysis of methadone extrinsic qualification were carried out from the perspective of gender.From Hongta District of Yuxi City,Yunnan Province,361 males and 67 females were treated with methadone.According to marital status,occupation,income and other indicators,14 women were included in the treatment,28 of them were male.After informed consent,a semi-structured interview method was used to conduct one-to-one in-depth interviews with key persons in a single room.The subjects completed the self-designed methadone maintenance service questionnaire at the same time.In addition,all the patients who took drugs from January 1,2014 to December 31,2017 in the CDC methadone clinic in Hongta District were selected as the study subjects,including 1156 men and 181 women.The study focused on demographic characteristics of users with different genders.The disease status and extroversion were compared and analyzed,as well as the logistic regression analysis of extrinsic factors.Results:There were 1156 males and 181 females,including 73 males,accounting for 6.3%of the male population and 15 females,accounting for 8.3%of the total female population.Although the difference was not statistically significant,on the basis of sex,the prevalence of different diseases was compared and analyzed.Only HIV positive and sex had statistical significance.It can be concluded that the positive rate of HIV in women is higher than that in men.Related studies have confirmed that gender is a factor in the high positive rate of HIV in women.The comparative analysis of different diseases based on marital status is statistically significant.There is no statistical significance in the comparative analysis of marital status and extrapolation.For both men and women,the rate of extrapolation was 8.5,higher than that of unmarried persons(5.1)and divorced or widowed(5.5).Because of the small sample size,there is no statistical significance in the analysis of small questionnaires,only description.Among the interview subjects,the male was 64.3%of the total male,the female was 50%of the total female,and the male outer-belt ratio was higher in the interviewee.A total of 12 of the interviewees had no income,accounting for 42.85%of the total.According to the Morisky compliance scale,only one male and eight women had good compliance.In the interviewees,the ratio of male outer-band was higher than that of female,but female compliance was higher than that of male,there was no statistical significance,only for reference.The related knowledge of safe sex was mainly the correct use of condoms.The awareness rate was generally lower than that of all the 5 questions answered correctly.There were 3 males and 1 female,which reflected the deficiency of health education in the health sector at present.From the gender point of view,the different roles and division of labor between men and women lead to different roles and norms of conduct.Women are always "good" and"obedient".Men are "rebellious","bold",and "have a wide circle of friends".And men have higher social status and greater peer pressure.The main factors affecting methadone maintenance compliance are economic cost,time the division of gender roles in society.From the point of view of marriage and children,people with children have a higher willingness to abstinence than married,childless and unmarried people.Women who assume the role of mothers have the highest compliance and withdrawal willingness.Drug side effects affected patients' confidence in treatment.Interviewees were mainly concerned about the side effects of methadone,such as obesity,decreased libido and other diseases.The service provider is mainly the service attitude of the health personnel and the public security department.At present,the biggest source of difficulty in maintaining methadone treatment is the conflict between the working concepts of the public security department and the health department.The public security department often goes to the methadone clinic for surprise checks,and those who have positive urine tests are sent directly to the strong rehabilitation center.The negative person of urine test because goes to the public security department to toss,not only wastes the time,but also affects the work and the normal life,most importantly,intensifies the contradiction between the public security department and the person under administration,increases the public security department law enforcement difficulty and reduces the public trust.Conclusion:There was no significant effect of gender in methadone extracellular service,but only tendentiousness effect,which focused on summing up and policy recommendations from the point of view of health services.The methadone extroversion service is a novel service,which is based on the negative rate of urine test.Whether or not to obtain extrinsic qualifications has become a motivating factor in stimulating compliance.Through the quantitative data analysis and qualitative data analysis,we found that the proportion of female outer-band is higher,and the methadone extroduction service has also promoted women to save time cost and take care of their families better in Hongta District,Yuxi City,Yunnan Province.In basic data analysis,the proportion of married people,regardless of male and female,was higher.In the current methadone extracycle service,there are still many problems.For example,price adjustment;potential methadone overdose;methadone knowledge dissemination,such as side effects,methadone withdrawal;lack of sexual health education in the health sector;and conflict between public security and health sector objectives.As methadone extracellular service is a relatively novel service,there is insufficient management,there are many areas for improvement.Specific suggestions are as follows:1.Those who are married and have children,regardless of whether they are male or female,should be properly relaxed.The price of methadone should be adjusted according to the patient's medication situation.For example,after a second admission to the group,after three months or six months of stable medication,To $5 per dose,to develop individualized treatment programs for those under treatment,to reduce unnecessary methadone overdose,to strengthen the management of methadone drugs,to reduce the diversion and illegal loss of methadone,and to strengthen health education and publicity in the health sector.It mainly introduces methadone maintenance therapy,side effects of methadone,methadone withdrawal and so on.For those under treatment,within the health sector,to expand to the wider community and to increase training in safe sex related health education,such as proper condom use,To reduce the incidence of sexually transmitted diseases(STDs)and improve the service attitude of health personnel,it is very important for them to change their attitudes,to promote the communication and cooperation between the health authorities and the public security departments,and to reduce the confrontation between the people under treatment and the public security departments. |
