Expression And Clinical Significance Of Galectin-3 And Chemerin In Patients With Type 2 Cardio-renal Syndrome

Objective:By observing the serum galectin-3 and chemerin levels in the type 2 cardio-renal syndrome,we preliminary explored the relationship between galectin-3 and chemerin in patients with type 2 CRS.Methods:A total of 150 chronic heart failure patients with HNYA II-IV class were recruited from January 2017 to December 2017 at the Department of Cardiology,Loudi Central Hospital.According to the estimated glomerular filtration rate,patients were divided into two groups:77 cases of type 2 CRS group with eGFR<60ml·min^-1·?1.73m²?^-1,and 73 cases of control group with eGFR?60ml·min^-1·?1.73m²?^-1.The type 2 CRS patients were grouped by eGFR:28 cases of CKD 3 group with 30?eGFR<60ml·min^-1·?1.73m²?^-1,27 cases of CKD 4 group with 15?eGFR<30ml·min^-1·?1.73m²?^-1,22 cases of CKD 5 group with eGFR<15ml·min^-1·?1.73m²?^-1.After the admission,at 6 o'clock in the morning on the following day,blood samples collected from the elbow vein were taken to detect galectin-3,chemerin,hs-CRP,etc.Results:1.the serum galectin-3 and chemerin levels were significant higher in patients with type 2 CRS than in those in control group,and the differences were statistically significant?10.28±4.92 VS 4.55±2.56ng/ml,p<0.05?,?4.62±1.05 VS 3.43±1.62ng/ml,p<0.05?.2.The serum galectin-3 and chemerin levels increased sequentially in CKD 3stage,CKD 4 stage,and CKD 5 stage,and the differences were statistically significant?7.10±3.31 VS 11.75±4.19 VS 14.59±4.16ng/ml,p<0.05?,?4.01±0.84 VS 4.91±0.79 VS 5.40±1.08ng/ml,p<0.05?.3.In the type 2 CRS group,The serum galectin-3 was positively correlated with hs-CRP?r=0.649,p<0.001?,The serum chemerin was positively correlated with hs-CRP?r=0.595,p<0.001?,The serum galectin-3 was positively correlated with chemerin?r=0.427,p<0.05?;4.The area under the curve of serum galectin-3 for diagnosing type 2CRS was 0.87?95%CI:0.805-0.939,p<0.05?,when its cut-off point was6.60ng/mL,the Youden index reached the maximum of 0.665,and then sensitivity and specificity of the prediction of type 2 CRS were 80.5%and 86.0%respectively;The area under the curve of serum chemerin for diagnosing type 2 CRS was 0.76?95%CI:0.664-0.855,p<0.05?,when its cut-off point was 3.92ng/mL,the Youden index reached the maximum of0.466,and then sensitivity and specificity of the prediction of type 2 CRS were 79.2%and 67.4%respectively.Conclusions:1.The serum galectin-3 and chemerin levels were elevated in type 2 CRS patients and correlated with their severity.2.The serum galectin-3 and chemerin have predictive value for diagnosis of type 2 CRS.
