Object To investigate the comparison of clinical effects between ultrasound-guided and landmark-guided subacmial-subdeltoid bursa(SASD)injections on the treatment of rotator cuff tears.Method 64 patients with rotator cuff tears were collected from the outpatient and inpatient department of the Second Affiliated Hospital of University of South China from March 2017 to October 2017.The basic group was treated with oral medicine combined with rehabilitation therapy, the ultrasound group was treated with anti-inflammatory analgesic drugs(dexamethasone sodium phosphate 5mg + 2%lidocaine2ml + saline 2ml)by ultrasound-guided SASD injections combined with oral medicine and rehabilitation therapy, the traditional group was treated with anti-inflammatory analgesic drugs(dexamethasone sodium phosphate 5mg + 2%lidocaine 2ml + saline 2ml)by landmark-guided SASD injections. Then,to evaluate the patient shoulder visual analogue scale(VAS),active flextion range of shoulder,University of California at Los Angelesof the shoulder joint before treatment, immediately aftertreatment,3 days after treatment,1 week and 2 weeks and 4 weeks and 6weeks after treatment.Result 1.There was no significant difference in age, sex,The average duration,VAS score, active flextion range of shoulder and UCLA score before treatment among the three groups.2.Compared with the one of other two groups respective, the VAS score of ultrasound group decreased at each time point after treatment, and there was a significant difference(P<0.05).3.The anterior flexion of the Ultrasound group increased at each treatment time, and there is a statistically significant difference between the ultrasound group and the basic group(P <0.05).Compared with the traditional group, there is a statistical difference among 1 week,2 weeks,4 weeks and 6 weeks after treatment(P <0.05).4.During the UCLA evaluation,"excellent" grade patients in the last follow-up ultrasound group were significantly more than the other two groups. Conclusion The treatment therapy of ultrasound-guided SASD injection with steroid drugs is better than landmark-guided SASD injection with steroid drugs and oral medicine combined with rehabilitation therapy, which is worth promoting in clinic.
